Septic Permit 555-18-007108-PRMT Page 4 of 5
Date Certificate:Issued.03/07/2019
Work Description . MAJ REP'FOR:5-BDRM-RES
Electrical Inspector. The work must be left uncovered and accessible for inspection. The
electrical inspection includes the connections to the pump and alarm in the dosing tank,
the wiring in the trench and the connections in the structure. The effluent pipe must have
18-gauge minimum green tracer wire or green metallic tape placed along the line.
TANK RISER: The tank must be installed with watertight risers extending to the ground
surface or above. The riser lid must be gasketed and securely fastened.
GRAVELLESS ABSORPTION TRENCHES ARE REQUIRED as part of this installation.
Hole spacing in the pressure distribution piping will be 24 inches on center or less. A
minimum 2-foot squirt height is required at the remotest orifice. Green tracer wire is
required above the piping and each line must end with a long sweep elbow and flushing
orifice with removable cap at or above finished grade. Each pressure lateral must have
at least one inspection port with minimum diameter of 4 inches. Chamber must be
supported by at least 4 inches of drain media (drainrock). A barrier to prevent
burrowing animals from entering the absorption chambers is required.
THE ATTACHED SEPTIC TANK ABANDONMENT FORM MUST BE COMPLETED AND
RETURNED TO OUR OFFICE. The septic system installer must complete and sign the
form and attach a copy of the receipt from the septic tank pumping service for pumping the
tank.
DRY SOIL CONDITIONS ARE REQUIRED FOR INSTALLATION(March 1 — October 31)
The geographical area in which the property lies is considered a risk to installation during
winter months. Soil moisture conditions generally preclude construction of the drainfield in
the winter months.
MAINTAIN PERMANENT ACCESS to septic tank for pumping and service. The septic tank
should be pumped and inspected by a licensed septic tank pumping service before sludge
and scum accumulations reach 30% of the tank liquid operating volume. Pumping every 3
to 5 years is an average interval. The septic tank is a reservoir for the solid materials in
the sewage waste stream. Excessive accumulation of these solid materials in the septic
tank will cause the solids to wash out of the tank and into the drainfield. This will cause
irreversible damage to the drainfield and will eventually lead to a system failure.
THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTEDfrom any vehicular
traffic, livestock and farming activities. Other activities such as excavating or filling,
driveway placement, paving or other similar activities that alter the natural soil conditions
will adversely affect the soil and the septic system function and lead to premature system
failure. The area must not be subjected to excessive saturation due to artificial drainage of
ground surfaces, driveways, building downspouts or other sources. The areas of the initial
septic system and the future repair system must both be protected.
3/7/19:4:27:40PM Page 4 of 5 ONS_OnsiteCSC.pr
