Septic Permit 555-18-007108-PRMT Page 3 of 5 <br /> Date Certificate Issuetl:.03/0.7/2019 <br /> Work Description, MAJ REP:FOR 5'BDRM RES <br /> Approved Area & System Type: <br /> An evaluation of the test holes dug for soil analysis was completed on September 19, <br /> 2018. The conditions observed during this site evaluation indicate the area around the <br /> evaluated test holes is not capable of supporting a standard septic system due to seasonal <br /> temporary groundwater level too close to the ground surface and limited usable area <br /> meeting setbacks. Please refer to the signed site plans dated September 28, 2018. <br /> Under these conditions, an ALTERNATIVE TREATMENT TECHNOLOGY (ATT) System <br /> with TREATMENT STANDARD #1 is required to adequately treat sewage effluent under <br /> DEQ rules. ATT Systems are devices connected to a septic system. They are designed to <br /> provide pre-treatment of effluent to reduce the level of bacteria prior to dispersal into a <br /> drainfield. They use mechanical circulation to provide a high level of treatment. ATT <br /> Systems are installed in soils where effective soil depth, saturated soils, available space, <br /> or other site conditions are severely limiting. <br /> This Permit is for a system designed to serve a single family dwelling with a maximum of 5 <br /> (five) bedrooms. Peak sewage flow into the system is limited to a maximum of 525 gallons <br /> per day, with an average sewage flow of not more than 263 gallons per day. Premature <br /> failure of the treatment system may occur if either of these flow quantities is exceeded. If, <br /> for some reason, you expect your domestic household water use may exceed these flows, <br /> it is advisable to increase the size of the treatment system. <br /> Specific Requirements: <br /> ATT Pretreatment Unit: Advantex AX25RTN as per submitted plans <br /> SERVICE CONTRACTS: The owner of an ATT system must maintain a service contract <br /> with a maintenance provider certified by the manufacturer to service and maintain the <br /> onsite system. A service contract must be entered into before the system is installed and <br /> must be maintained until the system is decommissioned. The service contract must <br /> provide for a minimum of four inspections and service visits by a maintenance provider <br /> scheduled once every six months over a two-year period to inspect, adjust, and service the <br /> ATT. The maintenance provider must submit an annual report and fee. Please review <br /> OAR 340-071-0345 for full requirements. <br /> ANTI-BUOYANCYis required for septic tank installation at this site. See manufacturer's <br /> specifications for anti-buoyancy. <br /> ELECTRICAL COMPONENTS ARE REQUIRED FOR INSTALLATION OF THIS SEPTIC <br /> SYSTEM. Detailed instructions for dosing tank/effluent pump installations are available in <br /> our office. The electrical wiring to the pump and alarm must be installed in accordance <br /> with the appropriate Electrical Code.
