File Number: 111784 <br />Page 3 of 8 Pages <br /> <br />SCHEDULE B <br /> <br />Minimum Monitoring and Reporting Requirements <br /> <br />System Monitoring Requirements <br />The permittee shall monitor the operation and efficiency of all treatment and disposal facilities. <br />Sampling and measurements taken as required herein shall be representative of the nature of the <br />wastewater, and shall be taken at peak usage during operation of the system. Unless otherwise agreed <br />to in writing by the Department of Environmental Quality, data collected, and submitted shall include <br />but not necessarily be limited to the following parameters and minimum ficequencies: <br /> <br />a. Influent to the Treatment Unit <br /> <br /> Item or Parameter <br />Flow, GPD <br /> <br />Flow Meter Calibration <br /> <br />Minimum Fr~?quen. cy <br />Monthly Average <br /> <br /> Annually <br /> <br />T~{pe of Sample <br />Measurement or calculation <br />based on meter readings <br /> <br />Verification <br /> <br />Effluent from the Treatment Unit to the Drainfield(s) <br /> <br /> Item or ParameterI Minimum Frequency I Type of Sample <br />BODs Semi-annually Grab <br />TSS Semi-annually Grab <br />NH3-N Annually Grab <br />NO3 Annually Grab <br />TKN Annually Grab <br /> <br />Operation and Maintenance Activities <br />The permittee shall record in writing all observations of operation and maintenance <br />activities, as required in the Department approved Operation and Maintenance Plan, on a <br />monthly basis. <br /> <br />Solids Management <br />The permittee shall maintain a record of the pumping dates and quantity (in gallons), of <br />solids/wastewater pumped, and what licensed sewage disposal service company pumped the <br />solids/wastewater, as well as the final disposal location and transfer locale (if applicable). <br /> <br />Reporting Procedures <br />Monitoring, maintenance practices, solids handling, and results shall be reported on Department- <br />approved forms. The reporting period is the calendar year. Reports must be submitted to the DEQ <br />office listed on the face page of this perm/t by January 15 following the reporting period. <br /> <br /> SCHEDULE D <br /> <br />Special Conditions <br /> <br />1. The permittee shall maintain on file a complete Operation and Maintenance (O&M) Plan approved by <br /> the Department. The permittee shall operate, manage and implement preventative maintenance <br /> <br /> <br />
