The <br />Department must approve any changes to the plan. <br /> <br />In the event that the a concentration limit, as specified in Schedule A, to the drainfield is exceeded, <br />the permittee shall within fourteen (14) working days of receipt of the analytical results: <br /> <br />a. Report the results to the Department; <br /> <br />b. Resample the effluent to the drainfield to verify the results; and <br /> <br />Sample the influent to the treatment unit for the following parameters and report the results to <br />the Department: <br /> <br /> Item or Parameter [ Type of Sainple <br />BOD5 Grab <br />TSS Grab <br />Grease and oils Grab <br />NI-t3-N Grab <br />NO3 Grab <br />TKN Grab <br /> <br />In the event that resampling the effluent confirms a concentration hmit violation or an influent <br />concentration limit listed in Schedule A 1. b) is exceeded, within thirty (30) days of <br />confirmation, the permittee shall submit to the Department a corrective action plan to reduce <br />the waste strength so that the concentration limits are not violated. Upon Department <br />approval, the plan shall be implemented by the permittee. <br /> <br />All septage/sludge shall be managed by a licensed sewage disposal service as defined in Oregon <br />Administrative Rule 340-71-100. <br /> <br />A deep-rooted, permanent grass cover shall be maintained on the drainfield area(s) at all times and <br />periodically cut to ensure maximum infiltration and evapotranspiration rate. <br /> <br />The drainfield area(s) including replacement area(s) shall not be subject to activities that would, in the <br />, opinion of the Department, adversely affect the soil or the functioning of the system. This includes, <br />but is not limited to, vehicular or animal traffic, filling or cutting, coveting the area with asphalt or <br />concrete, or subjecting the area to excessive saturation. <br /> <br />The permittee shall not be required to perform a formal hydrogeologic characterization or preliminary <br />groundwater monitoring during the term o£this permit provided that the facilities are operated in <br />accordance with the permit conditions, and there are no apparent adverse groundwater quality impacts <br />(compla'mts or other indirect evidence) resulting from the facility's operation. If warranted, the <br />Department may evaluate the need for or require a full assessment of the facility's impact on <br />groundwater quality and if necessary may reopen this permit to include groundwater monitoring <br />parameters. <br /> <br />An adequate contingency plan for prevention and handling of spills and unplanned discharges shall be <br />in force at all times. The permittee shall immediately notify the DEQ office listed on the face page of <br /> <br /> <br />
