NOTICE TO MORTGAGEE, LIENHOLDER, VENDOR, OR SELLER: ORS CHAPTER <br />215 REQUIRES THAT IF YOU RECEIVE THIS NOTICE, IT MUST PROMPTLY BE <br />FORWARDED TO THE PURCHASER. <br />NOTICE OF DECISION <br />URBAN PARTITIONING CASE NO. 04-02 <br />APPLICATION: Application of Billy Gostevskyh, Kahinika Shadrin, and Zinovei Shadrin to <br />divide a 20,578 square foot parcel into three parcels containing approximately 6,560, 6560, and <br />7,438 square feet in a SINGLE-FAMILY RESIDENTIAL (RS) zone located at 4111 Haysville <br />Drive NE, Salem. (T7S; R2W; Section 6CA; Tax lot 2400) <br />DECISION: Notice is hereby given that the Planning Director for Marion County has <br />APPROVED the above described Partitioning application subject to certain conditions. <br />EXPIRATION DATE: This approval is valid only when the final partitioning plat is recorded <br />by FEBRUARY 24, 2006. After recording the final partitioning plat, the applicant shall <br />provide proof of the recording to the Planning Division. This can be either a copy of the <br />recording fee receipt or a copy of the plat showing the County Clerk's stamp. The effective <br />period of an approved application may be extended for an additional year subject to approval of <br />an extension (Extension form available from the Planning Division). Additional extensions <br />may not be granted if the regulations under which this decision was granted have changed <br />since the original approval. <br />WARNING: A decision approving the proposed division or use is for land use purposes only. <br />To be sure the subject property can accommodate the proposed use the applicant needs to check <br />with the Building Inspection Division, (503) 588-5147. <br />This decision does not include approval of a building permit. <br />CONDITIONS: The following conditions must be met before a building permit can be obtained <br />or the approved use established: <br />1. Prior to the issuance of building permits, the applicant shall submit a final partitioning <br />plat to the County Surveyor's Office (5155 Silverton Road NE). Following plat approval, <br />it shall be recorded with the Marion County Clerk (final plat instructions and example <br />enclosed). <br />2. The applicant is advised that a Partition Plant Service Report, from a title company, will <br />be required upon submission of the final mylar to the County Surveyor. <br />3. Prior to the issuance of building permits, the applicants shall meet the Marion County <br />Public Works conditions listed in finding #5 of this report. The conditions shall be met to <br />the department's satisfaction. <br />4. The Marion County Planning Division, in coordination with the 9-1-1 Emergency System <br />has named the proposed private access easement Tree Lane NE, consistent with the <br />
