Please refer to the signed approved site plan
dated August 27, 2015. Any deviation from the approved plans must be authorized by this office prior to
installation or the permit may be voided
SEPTIC TANK: 1000 gallons
EFFLUENT SEWER: The effluent sewer is the pipe from the septic tank to the distribution box. The
material required is 3034 or Schedule 40 pipe with 18-gauge minimum green tracer wire or green metallic
tape placed along the pipe. The effluent sewer shall extend at least five(5) feet from the tank. There must
be a minimum difference of eight(8) inches between the invert(flow line) of the septic tank outlet and the
invert of the distribution or drop box outlet. In Equal Distribution, there must be six (6) inches of fall
between the septic tank outlet and the distribution box inlet. In Serial Distribution, there must be two (2)
inches of fall between the septic tank outlet and the drop box inlet.
TANK RISER: The tank must be installed with at least one watertight riser extending to the ground
surface or above. The riser lid must be gasketed and securely fastened.
OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the DEQ rules. A
copy of the DEQ setback requirements is on file in our office and available online at
http://www.deq.state.orus/wq/onsite/sdssearch.asp under Rules: Division 71: Table 1.
THE ATTACHED SEPTIC TANK ABANDONMENT FORM MUST BE COMPLETED AND
RETURNED TO OUR OFFICE. The septic system installer must complete and sign the form and attach
a copy of the receipt from the septic tank pumping service for pumping the tank.
THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic or other
sources of soil compaction. Activities such as excavating or filling, driveway placement, paving or any
similar alteration of the natural soil conditions will adversely affect septic system function and lead to
premature system failure. The area must not be subjected to excessive saturation due to artificial drainage
of ground surfaces, driveways, building downspouts or other sources. The areas of the initial septic
system and the future repair system must both be protected.
THIS SYSTEM MUST BE INSPECTED AND APPROVED PRIOR TO BACKFILL. When work is
complete and ready for inspection, request an inspection by following the instructions on the
accompanying permit document. Be sure the System Record form with the As-Built drawing is on the job
or in our office when the inspection is requested. An inspection should be completed within seven (7)
days.
5155 Silverton Road NE•Salem.OR 97305-3802•y.ww.co marion.or us
Printed on recycled paper• Reduce-Reuse-Recycle-Recover
Page l of l r fl
1/r
