Please refer to the signed approved site plan <br /> dated August 27, 2015. Any deviation from the approved plans must be authorized by this office prior to <br /> installation or the permit may be voided <br /> SEPTIC TANK: 1000 gallons <br /> EFFLUENT SEWER: The effluent sewer is the pipe from the septic tank to the distribution box. The <br /> material required is 3034 or Schedule 40 pipe with 18-gauge minimum green tracer wire or green metallic <br /> tape placed along the pipe. The effluent sewer shall extend at least five(5) feet from the tank. There must <br /> be a minimum difference of eight(8) inches between the invert(flow line) of the septic tank outlet and the <br /> invert of the distribution or drop box outlet. In Equal Distribution, there must be six (6) inches of fall <br /> between the septic tank outlet and the distribution box inlet. In Serial Distribution, there must be two (2) <br /> inches of fall between the septic tank outlet and the drop box inlet. <br /> TANK RISER: The tank must be installed with at least one watertight riser extending to the ground <br /> surface or above. The riser lid must be gasketed and securely fastened. <br /> OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the DEQ rules. A <br /> copy of the DEQ setback requirements is on file in our office and available online at <br /> http://www.deq.state.orus/wq/onsite/sdssearch.asp under Rules: Division 71: Table 1. <br /> THE ATTACHED SEPTIC TANK ABANDONMENT FORM MUST BE COMPLETED AND <br /> RETURNED TO OUR OFFICE. The septic system installer must complete and sign the form and attach <br /> a copy of the receipt from the septic tank pumping service for pumping the tank. <br /> THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic or other <br /> sources of soil compaction. Activities such as excavating or filling, driveway placement, paving or any <br /> similar alteration of the natural soil conditions will adversely affect septic system function and lead to <br /> premature system failure. The area must not be subjected to excessive saturation due to artificial drainage <br /> of ground surfaces, driveways, building downspouts or other sources. The areas of the initial septic <br /> system and the future repair system must both be protected. <br /> THIS SYSTEM MUST BE INSPECTED AND APPROVED PRIOR TO BACKFILL. When work is <br /> complete and ready for inspection, request an inspection by following the instructions on the <br /> accompanying permit document. Be sure the System Record form with the As-Built drawing is on the job <br /> or in our office when the inspection is requested.
