Joye,REHS, Onsite Wastewater Specialist <br /> ONSITE WASTEWATER TREATMENT SYSTEM MINOR REPAIR <br /> TANK REPLACEMENT <br /> The proposed septic tank installation is approved as submitted. Please refer to the signed <br /> approved site plan dated March 27, 2013. Any deviation from the approved plans must be <br /> authorized by this office prior to installation or the permit may be voided <br /> A failing system must be immediately repaired to eliminate the public health hazard per OAR <br /> 340-071-0215. <br /> EFFLUENT SEWER:The effluent sewer is the pipe from the septic tank to the distribution or drop box. <br /> The material required is 3034 or Schedule 40 pipe with 18-gauge minimum green tracer wire or green <br /> metallic tape placed along the pipe. The effluent sewer shall extend at least 5-feet from the tank. There <br /> must be a minimum difference of 8-inches between the invert(flow line)of the septic tank outlet and the <br /> invert of the distribution or drop box outlet. In Equal Distribution, there must be 6-inches of fall <br /> between the septic tank outlet and the distribution box inlet, and 2-inches of drop within the box. In <br /> Serial Distribution,there must be 2-inches of fall between the septic tank outlet and the drop box inlet, <br /> and 6-inches of drop within the box. <br /> OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the DEQ rules. The <br /> well placement is especially critical and must be located at least 50-feet from the septic tank. A <br /> minimum 5-foot setback is required from the septic tank to a building foundation. A copy of the DEQ <br /> setback requirements is available in our office. <br /> THE ATTACHED SEPTIC TANK ABANDONMENT FORM MUST BE COMPLETED AND <br /> RETURNED TO OUR OFFICE.The septic system installer must complete and sign the form and attach <br /> a copy of the receipt from the septic tank pumping service for pumping the tank. <br /> THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, <br /> livestock and farming activities. Other activities such as logging, excavating or filling, driveway <br /> placement or any similar activities may also damage the soil in the area and adversely affect the septic <br /> system approval.
