The proposed system <br /> stakeout has been approved and installation plans have been approved as submitted. Please refer to the <br /> signed approved site plan dated July 9,2018. Any deviation from the approved plans must be authorized by <br /> this office prior to installation or the permit may be voided. The required inspection schedule is attached to this <br /> permit. It's the responsibility of applicant/installer to coordinate these requirements with all others in OAR <br /> 340-071-0220. <br /> This Permit is for a system designed to serve a single family dwelling with a maximum of four(4)bedrooms. <br /> Peak sewage flow into the system is limited to a maximum of 450 gallons per day,with an average sewage flow <br /> of not more than 225 gallons per day. Premature failure of the treatment system may occur if either of these <br /> flow quantities is exceeded. If,for some reason,you expect your domestic household water use may exceed <br /> these flows, it is advisable to increase the size of the treatment system. <br /> Specific Requirements <br /> ATT Pretreatment Unit: Advantex AX20-RT as per approved attached plan. <br /> SERVICE CONTRACTS: The owner of an ATT system must maintain a service contract with a maintenance <br /> provider certified by the manufacturer to service and maintain the onsite system. A service contract must be <br /> entered into before the system is installed and must be maintained until the system is decommissioned. The <br /> service contract must provide for a minimum of four inspections and service visits by a maintenance provider <br /> scheduled once every six months over a two-year period to inspect,adjust,and service the ATT. The septic <br /> tank must be pumped before solids reach 30%of the tank liquid operating level. The maintenance provider <br /> must submit an annual report and fee. Please review OAR 340-071-0345 for full requirements. <br /> DRY SOIL CONDITIONS ARE REQUIRED FOR INSTALLATION(March 1 —October 31) The geographical <br /> area in which the property lies is considered a risk to installation during winter months. Soil moisture conditions <br /> generally preclude construction of the drainfield in the winter months. <br /> GRAVELLESS ABSORPTION TRENCHES ARE REQUIREDas part of this installation. Hole spacing in the <br /> pressure distribution piping will be 24 inches on center or less. A minimum 2-foot squirt height is required at <br /> the remotest orifice. Green tracer wire is required above the piping and each line must end with a long sweep <br /> elbow and flushing orifice with removable cap at or above finished grade. Each pressure lateral must have at <br /> least one inspection port with minimum diameter of 6 inches. Chamber must be supported by at least 4 <br /> inches of drain media(drainrock)to provide adequate footing. A barrier to prevent burrowing animals from <br /> entering the absorption chambers is required. <br /> TANK RISER: The tank must be installed with watertight risers extending to the ground surface or above. The <br /> riser lid must be gasketed and securely fastened. <br /> ANTI-BUOYANCY is required for septic tank and ATT pod installation at this site. See manufacturer's • <br /> specifications for anti-buoyancy. <br /> AN EFFLUENT LIFT PUMP WILL BE REQUIRED to install the septic system disposal field in the approved <br /> area and at the required depth. Detailed instructions for dosing tank/effluent pump installations are available in <br /> our office. The electrical wiring to the pump and alarm must be installed in accordance with the appropriate <br /> Electrical Code. The electrical work must be done under a Marion County Electrical Permit and must be <br /> inspected and approved by a Marion County Electrical Inspector.
