SECTION 01313 <br />DEMOLITION SUBMITTALS <br />1q9~ PAGE 1 OF 3 <br />1.1 SUBMITTALS AND NOTICES <br />The following items shall be submitted to the Owner or Owner's Representative for review and approval <br />prior to beginning of work: <br />Survey: Before any work is started, the Contractor shall make a thorough survey with the Owner or <br />Owner's Representative in which alterations occur and areas which are anticipated routes of access, and <br />fu ~sh a report, signed by both, to the Contracting Officer. This report shall be listed by building: <br />1. Existing condition and types of surfaces not required to be altered throughout the areas effected. <br />~ 2. Shall designate areas for working space, materials storage and routes of access to areas witfun <br />buildings where demolition occur. <br />3. One week prior to th~beginning of the work the Contractor shall provide a written, project-specific <br />work plan signed by Professional Surve in Services" company for the review and approval of <br />the Owner's Representative. is wor p an s all include the number of workers that will be <br />assigned to given work areas, as well as the sequence of operations and anticipated duration or <br />work at specific areas. This plan shall also include a diagram showing the locahons of the work <br />areas. <br />~ 4. A copy of the Contractor's written respirator program that complies with all parts of OSHA <br />Asbestos Regulations CFR Title 29, Part 1910, Section 1910.1001. <br />~ 5. A copy of the Contractor's certificate for general liability insurance naming the Owner and his <br />Agents by an "Add~tional Insured Endorsement". <br />~ 6. A co of the Contractor's license. <br />PY <br />r- ; <br />_~ 7. Copies of all pernuts necessary to execute and complete the work of this project. <br />% <br />8. Evidence that all necessary pernuts have been acquired and arrangements mad for he trans~rt <br />and disposal of demolition materials to a site ap,proved bv the Owner or Owner's Representative. _ <br />When necessary, the Contractor shall obtain and submit to the Owner's Representative the specific <br />written approval of the Oregon Department of Environmental Quality (and/or other agency having <br />jurisdiction) for the equipment or work techniques to be used. <br />9. Environmental Protection Plan: After the contract is awarded, prior to the commencement of the <br />work, the Contractor shall meet with the Owner or Owner's Representative to discuss the proposed <br />Environmental Protection Pian and to develop mutual understanding relative to details of <br />environmental protection. Not more than 10 days after the meeting, the Contractor sha11 prepaze <br />and submit to the Owner or Owner's Representative for approval, a written and/or graphic <br />Environmental Protection Plan including, but not limited to, the following: <br />a. A list of Federal, State and L,ocal laws, regulations and permits conceming environmental <br />protection, pollution control, noise control and abatement that are applicable to the <br />Contractor's proposed operations and the requirements imposed by those laws, regulations <br />imposed by those laws, regulations and permits. <br />
