~ <br />Workers' Compensation <br />'+~.rr, <br />Provide proof to Marion County that the Contractor is registered <br />by the Oregon Workers' Compensation Division either as a carrier- <br />insured employer, a self-insured employer, or is an independent <br />contractor who will perform the work on the project without the <br />assistance of others. Proof will be required before the Board of <br />Commissioners will execute the contract. <br />Certificate(s) of Insurance <br />Prior to commencing work, the Contractor shall provide a <br />Certificate evidencing the insurance required by the Contract and <br />a separate endorsement adding Marion County, its officials, <br />agents, employees, and volunteers as insured. The Certificate <br />shall state that coverage afforded the County as an Insured shall <br />apply as primary and not excess to any insurance issued by the <br />County, provide a Cross Liability Clause, and state that the <br />Contractor is responsible for payment of all insurance <br />deductibles on the above-described policies. <br />Pre-bid Conference <br />A mandatory pre-bid conference will be held on September 15, 1997 <br />at 2:00 PM, Conference Room "B", 5th Floor, Marion County <br />Courthouse, 100 High St. N.E., Salem, Oregon, 97301. Those <br />interested in attending the pre-bid conference must make advance <br />arrangements by contacting Marion County Risk Management at (503) <br />588-5294 no later than 5:00 PM September 11, 1997. <br />Preconstruction Conference <br />A preconstruction conference will be lield Wednesday, October 8, <br />1997 in Room 1330, lst Floor, Marion County Courthouse, 100 High <br />Street NE, Salem, Oregon, 97301 at 1:30 PM. <br />Failure to Complete on Time and Liquidated Damages <br />In the event that the CONTRACTOR should fail to complete the work <br />under this contract by the specific time of completion or any <br />extensions thereof, the CONTRACTOR shall pay to the OWNER as <br />fixed, agreed and liquidated damages the sum of $500 for each <br />calendar day of delay. <br />
