Category 1: Unable to board, disembark, or ride regular accessible buses and/or navigate the <br />system. <br />Category 2: Able to use accessible bus, but accessible bus not available. <br />Category 3: Unable because of a disability to travel to or from a regular bus stop. (Those in Category <br />3 will be transported to the nearest practical bus stop and from a bus stop to their point of <br />origin.) <br />CONTRACTOR shall provide program participants with identification cards, a registration number, <br />expiration date, eligibility category, need for Personal Care Attendant (PCA), and any other conditions <br />under which a person may use the CherryLift. When a person is able to use the bus for some of their <br />trips, then conditional eligibility is granted. Temporary eligibility is granted to those whose condition is <br />expected to improve and is indicated by the cards color and expiration date on the ID Card. Except for <br />temporary eligibility, all identification cards shall expire three years from its date of issue or at such <br />earlier times as DISTRICT determines that the individual is no longer eligible. <br />First time applicants will be allowed to receive services on a temporary basis for up to 21 days prior to <br />the completion of their application. If a completed application is not received within 21 days the <br />applicant will be notified of the pending discontinuation of the temporary service. <br />The CONTRACTOR shall provide visitors with disabilities, who reside outside of the DISTRICT, <br />CherryLift trips. CONTRACTOR shall require visitors to present an ADA Eligibility Card that certifies <br />their eligibility within the jurisdiction they reside. CONTRACTOR shall require visitors without an ADA <br />Eligibility Card to present documentation of their residence and disability. CONTRACTOR shall require <br />that ~-visitors without documentation ts-submit a completed self-certification application. <br />CONTRACTOR shall provide CherryLift service for up to 21 days to any visitor with a disability. <br />The CONTRACTOR is responsible for certifying individuals for this program and the DISTRICT <br />reserves the right to expand or reduce the number of program participants and to modify the method(s) <br />and standard(s) used for certification. Each program participant is listed in the CONTRACTOR's client <br />file and the DISTRICT will pay only for trips made by the program participants_ , <br />2.2 CherryLift Service Components <br />CherryLift is a curb-to-curb advance reservation service operated, contracted, brokered, or othennrise <br />coordinated by the CONTRACTOR on behalf of the DISTRICT. <br />With each trip request, ~e CONTRACTOR will first determine if the program participant is certified and <br />determine if th specified trip is eliqible. Once a program participant has been <br />determined eligible for the specified trip, the CONTRACTOR will match the program participant trip <br />with the most efficient and cost effective service that is available and which will meet the ADA service <br />standards.
