This schedule, unless objected to by the Owner ~s#i~es~, sha(1 be used as a basis for reviewing the Contractor's Applications for Payment. 9.3 APPLICATIONS FOR PAYMENT 9.3.1 At least five 5 davs before the last dav of the pavment eriod for each progress payment, the Contractor shall submit to the Architect and Construction Re~resentative itemized Application for Payment for operations completed in accordance with the schedule of values. Such application shatl be notarized, if required, and supported by such data substantiating the Contractor's right to payment as the Owner or Architect may require, such as copies of requisitions from Subcontractors and material suppliers, and reflecting retainage if provided for elsewhere in the Contract Documents. _ Such Aoalication for Pavment shall include an itemization of Work comoleted in accordance with the Contract Documents. Such A~alication for Pavment shall be certified as correct~ Contractor and shal! be accom~anied by the form of Affidavit, Lien Waiver and Release set forth in the attachment to the Contract Documents excludin aarasraah 3 thereo for work nerformed throueh the _prior billine month. The Contractor's aoplication for final ~avment shal! similarlv be _accomnanied ~ the attached Affidavit. Lien Waiver and Release, includine paraeraph 3 thereof.
