This schedule, <br />unless objected to by the Owner ~s#i~es~, sha(1 be used as a <br />basis for reviewing the Contractor's Applications for <br />Payment. <br />9.3 APPLICATIONS FOR PAYMENT <br />9.3.1 At least five 5 <br />davs before the last dav of the pavment eriod for each <br />progress payment, the Contractor shall submit to the <br />Architect and Construction Re~resentative itemized <br />Application for Payment for operations completed in <br />accordance with the schedule of values. Such application <br />shatl be notarized, if required, and supported by such data <br />substantiating the Contractor's right to payment as the Owner <br />or Architect may require, such as copies of requisitions from <br />Subcontractors and material suppliers, and reflecting <br />retainage if provided for elsewhere in the Contract <br />Documents. _ Such Aoalication for Pavment shall include an <br />itemization of Work comoleted in accordance with the <br />Contract Documents. Such A~alication for Pavment shall be <br />certified as correct~ Contractor and shal! be accom~anied <br />by the form of Affidavit, Lien Waiver and Release set forth in <br />the attachment to the Contract Documents excludin <br />aarasraah 3 thereo for work nerformed throueh the _prior <br />billine month. The Contractor's aoplication for final ~avment <br />shal! similarlv be _accomnanied ~ the attached Affidavit. <br />Lien Waiver and Release, includine paraeraph 3 thereof. <br />Anv artial and the final navment re uest shall also be <br />accompanied ~ forms of subcontractor/suaalier A~davit, <br />Lien Waiver and Release as set forth in the attachment to the <br />Contract Documents si ned ~ the subcontractors and <br />suapliers of~ tier on the Pr_ oiect, as mav be reauested~ <br />Owner. _ In addition, such Application for Pavment shall <br />contain a certification ~ the Contractor that there are no <br />written claims of nonpavment or of inechanics' or <br /> <br />Owner-Contractor Agreement General Conditions Supplement (For AIA Doc.
