SALEM COURTHOUSE SQUARE <br />DEMOLITION <br />1997 <br />SECTION 00200 <br />INSTRUCTIONS/RFP <br />PAGE 1 OF 1 <br />INSTRUCTIONS TO CONTRACTORS <br />201 Each proposer must attend the mandatory walk-through at the time and place specified in the Invitation <br />to be eligible for contract award. <br />202 Prior to submitting a proposal, each contractor must carefully examine the requirements of the Proposal <br />Documents and become fully familiar with site conditions affecting the work. <br />203 Each contractor must verify quantities and locations of materials to be abated and rely on this <br />examination in preparing the proposal. <br />204 All proposals shall be of a single fixed price and shall be cited in both written and numeric form. In <br />cases of discrepancies between the amounts, the written amount sha11 take precedence. <br />205 Each proposal sha11 be submitted on the form provided as Section 0300 of the RFP Documents, and shall <br />be typed or neatly handwritten in ink. Proposals shall be fully completed and shall contain no deletions, <br />alterations or recapitulations of the work to be performed. <br />206 Each proposal must be signed by a duly authorized individual with the person's usual signature. <br />Proposals by partnerships must bear the signature of one of the partners. <br />207 A proposal may be modified or withdrawn by an appropriate document duly executed (in the manner <br />that a proposal must be executed) and delivered to the place where proposals aze to be submitted at any <br />time prior to the time set for receipt. <br />208 Each proposal shall be submitted in an opaque sealed envelope and delivered at the time and place <br />identified in the "Invitation/RFP." Each proposal envelope shall show the proposer's name and address, <br />and the project title. <br />209 The selected contractor is required to pay a fee to the Bureau of labor and Industries pursuant to the <br />provisions of ORS 279352(2). The fee is one-tenth of one percent of the price of this contract, but not <br />less than $100 nor more than $5,000, regazdless of the contract price. <br />210 Each proposal must include a package 1 for a project with a six (6) week completion period and package <br />2 for a project with an eight (8) week completion period, or both. Each package should be completed on <br />the forms provided. <br />
