Proposals shall include the following: <br />1. Proposed fee schedule, including optional services, if any <br />2. A list of team members and their qualifications and titles <br />3. Which team member(s) will be on-site <br />4. Provide at least one (1) reference from: architect, contractor and owner <br />5. Experience and knowledge in managing projects of this type and size <br />6. Experience and ability to work in a team environment <br />7. Briefly discuss your ability to manage this project and what unique skills and <br />talents you, and/or your team, offer the owners, if selected <br />8. Briefly describe the control methods, including computer systems and hardware, <br />available to track and monitor budgets, schedules and project complerion <br />9. A not to exceed budget for a11 reimbursable costs <br />10. The closest branch office andlor main office location if applicable <br />1 l. How will job-site office be established and who will be responsible for cost <br />12. What date are you available to start <br />13. Include a sample of the contract for proposed PM/CM duties <br />Date for Submission of Proposals: <br />Proposers sha11 submit e 3 copies of the proposal to: <br />Marion County Clerk's Office <br />Marion County Courthouse <br />100 High St. NE <br />Salem Oregon, 97301 <br />All proposals must be received no later than 3:00 P.M, May 19, 1997.
