Based on the results of this study, site <br />approval was granted. The approval is site specific to the area tested, and the septic <br />system must be located in the immediate area of the test pits, unless otherwise authorized <br />by this office, A new site evaluation may be required if the system is proposed for a <br />different area on the property. <br /> <br />This approval was given on the basis that the lot or parcel will not be further partitioned <br />or subdivided and that conditions on this property and on adjacent properties are not altered <br />in any manner which would prohibit the issuance of a permit, Soils in the approved area <br />shall not be altered by cutting, filling, or other developmental activities which would <br />impede the successful operation of the initial or replacement absorption systems. Failure <br />to comply with the above items may void this approval. <br /> <br />The site evaluation approval runs with the land, but the permit is non-transferable and non- <br />refundable. <br /> <br />Keep this document with your records, Show this permit to the septic installer prior to <br />installation of the system, Any deviation from the approved plan must have prior approval from <br />this office, <br /> <br />The following construction specifications shall apply to the installation of the initial <br />septic system: <br /> <br />TYPE OF INITIAL SYSTEM: CAPPING FILL <br /> <br />MINIMUM SEPTIC TANK CAPACITY: t000 GALLONS <br /> LINEAL FEET OF DPJ~INFIELD: 180 <br /> TYPE OF DISTRIBUTION: EQUAL <br /> DISPOSAL TRENCH DEPTH: 12INCHES, MINIMUM; 12INCHES. MAXIMUM <br /> TOTAL DPJ~IN MEDIA DEPTH: 12INCHES <br /> DRAIN MEDIA UNDER PIPE: 6INCHES <br /> MINIMUM SOIL BACKFILL: 12INCHES <br /> GROUND WATER INTERCEPTOR: N <br /> <br />The disposal trenches are constructed two (2) feet wide and on minimum ten (10) foot centers <br />and with no line exceeding 125 feet in length. The trenches and perforated pipe are <br />installed level (+/- one inch) and contoured to follow the natural ground surface, <br /> <br />TYPE OF FU~RE REPLACEMENT SYSTEM: CAPPING FILL <br /> <br />COMMEkFFS AND CONDITIONS <br /> <br />1. THIS SEPTIC REVIEW WAS COMPLETED BY Z,EWING ON 6/16/98. THE <br /> SOIL IN THE TEST PITS INDICATE A WINTER WATER TABLE OF 12"T0 <br /> 16"FROM THE GROUND SURFACE,DUE TO THE SEVERE WATER TABLES ON <br /> MIS SITE A CAPPING FILL DRAINFIELD WITH A TILE DEWATERING <br /> SYSTEM IS APPROVED AS SHOWN ON THE REVISED SITE PLAN DATED <br /> 7/8/98. THE CAPPING FILL MATERIAL MUST BE APPROVED PRIOR TO <br /> INSTALLATION, A PRECONSTRUCTION MEETING ONSITE IS RECOMMEND- <br /> ED TO AVOID POSSIBLE CONFLICTS, <br /> THIS APPROVAL IS GPJNNTED USING THE RURAL AREA CONSIDEPJ~TION <br /> RULES ALLOWED BY THE'DEPARTME~FF OF ENVIRONMENTAL QUALITY. <br />2. THE ATTACHED OPJ~NGE CARD MUST BE POSTED AT THE COUNTY ROAD <br /> prior to requesting an inspection, The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site, <br />2. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any <br /> vehicular traffic, livestock and farming activities, Other <br /> activities such as logging, excavating or filling, driveway <br /> placement or any similar activities may also damage the soil <br /> in the area and adversely affect the septic system approval. <br /> The areas of the initial septic system and the future repair <br /> system must both be protected. <br />4, A CAPPING FILL DRAINFIELD IS REQUIRED in conjunction with <br /> <br /> <br />
