Based on the results of this study, site evaluation <br />approval ~s granted, The approval is site specific to the area tested, and the septic system <br />must be located Jn the immediate area of the test pits, unless otherwise authorized by this <br />office. A new slte evaluatlon may be required if the system is proposed for a different <br />area on the property. <br /> <br />This approval Js given on the basis that the lot or parcel will not be further partitioned <br />or subdivided and that conditions on this property and on adjacent properties are not altered in <br />any manner whqch would prohibit the issuance of a permit, Soils in the approved area shall not be <br />altered or modified by cutting, filling, or other developmental activities which would impede the <br />successful operation of the initial or replacement so~l absorption systems. Failure to comply with <br />the above ~tems may void this approval, <br /> <br />This report is not a permit to install the sewage system. A permit must be obtained before <br />any system construction occurs. A permit may be issued only after the property has been found to <br />be compatible with applicable LCDC acknowledged local comprehensive land use plan, A Statement of <br />Land Use CompatibJlity or its equivalent is required before a construction permit can be issued. <br /> <br />A detailed site plan. drawn to scale (ex: 1" = 60"). must be submitted with the permit <br />application before the construction-installation permit can be issued. The system must be <br />installed by the property owner or a reglstered contractor. <br /> <br />Thqs site evaluation report is valid until an on-site sewage system is installed pursuant to the <br />construction permit obtained from this office, or until cancelation pursuant to the Environmental <br />QualJty Commission Rules. A PRE-PERMIT FIELD STAKE <br /> OUT IS NECESSARY TO CONFIRM THE LOCATIONS AND ELEVATIONS OF <br /> THE INDIVIDUAL DRAINFIELD LINES, REFER TO THE STAKEOUT PRO- <br /> CEDURE AND SETBACK LIST FOR DETAILS TO COMPLETE THE STAKEOUT <br /> USE CARE NOT TO ALTER THE NATURAL TOPSOIL WITHIN THE DRAIN- <br /> FIELD SITES PRIOR TO INSTALLATION, NOTE THE DRY SOIL SUMMER <br /> SEASON CONSTRUCTION LIMITATIONS. HOLD OFF SITING THE WELL <br /> PENDING THE STAKEOUT REVIEW AND APPROVAL. <br />2. THE ATTACHED ORANGE CARD MUST BE POSTED AT THE COUNTY ROAD <br /> prior to requesting an inspection, The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site, <br />3.
