My <br />understanding is the submittal was a prelinfinary engineered diagram. <br /> <br />2) <br /> <br />The plans used for the eo~ruction were prepared by Zan Ewing who was a consultant <br />and is currently employed by Marion County. He is no longer involved with the facility. <br />The facility is clearly not installed or functioning as designed. The certification you <br />submitted can't be accepted due to the deficiencies noted above. <br /> <br />3) <br /> <br />The plans specified a 2,000 gallon recircuiation tank, a 1,000 gallon tank was installed. <br />This is a violation of Oregon Administrative Rules (OAR) 340-071-0302(4)(a) which <br />requires the tank volume equal the design flow. Class II Violation <br /> <br />4) <br /> <br />The splitter assembly was not installed as per manufa~er's specifications due to the inlet <br />tee of the recirculation tank being in the way. The hydrosplitter was installed upside dowa <br />and fed by gravity flow, not in accordance with the manufacturer's specifications. This is <br />a violation of OAR 340-071-0130(21) which requires all materials and equipmeut ha mx <br />on-site system to be installed and maintained in accordance with the manufacturer's <br />specifications. Class II Violation <br /> <br />5) <br /> <br />The pressure laterals on the gravel filter are to be covered by drain media to prevent aerial <br />misting of the partially treated sewage. This is a pubhc health hazard and a nuisance. <br />This is a violation of OAR 340-071-0302(3)(c) which requires adequate cover over the <br />laterals to prevent aerial spray drift. Class II Violation <br /> <br />6) <br /> <br />Schedule D(6) requires an approved Operation & Maintenance (O&M) plan to be <br />maintained at the facility. To date, we have not received an O&M plan for rev/ew and <br />approval. OAR 340-052-040(4) specifies the design engineer should prepare the O&M <br />plan.
