The Semis system is proposed to serve a three <br />bedroom residence on the prop~z'ty. <br /> <br /> Based on the results of this study, this area does not appear suitable <br />for a standard on-site sewage system. However, the site appears suitable for <br />the installation of an alternative SAND FILTER PLUS FLr~L ~APFING FILL SYSTEMS <br />as described in the attached construction detail and setback sheets. <br /> <br /> This approval is given on the basis that She lot OF parcel will not b~ <br />further partitioned er subdivided ar~ that conditions on this property a~d on <br />adjacent p~eperties are not altered in any manner which would prohibit the <br />issuance of a pe~mit. Soils in the a~roved area shall not be altered or <br />modified by cutting, filling~ or other develo~sntal activities which would <br />impede the SUCCessful opera, ion of the initial or replacement soil ab~o=ption <br />systems. Failure to comply with the above items may void this approval. <br /> <br /> ~is report ia not a De=mit to insts3_l the s~wa~e ~¥~tem. A pe~it must <br />be obtaine~b$fore any ~¥ste~ ¢onstru~ion occurs. A permit ~ay be issued only <br />after the property has been found to be c~patible with applicable LCDC <br />acknowledged local comprehensive land use plan. A State~ent of Land Use <br />Compatibility or its equivalent is required before a construction permit can <br />be issued, <br /> <br /> A detailed plot plan, draw~ to scale (ex. 1, = 60'), must be submitted <br />with the permit application before the construction-installation permit can be <br />issued. The system must be installed by the property owner Or a DEQ licess~d <br />septic installer. <br /> <br /> This site evaluation report is valid nntil an on-site sewage system is <br />installed pursuant to the const~uctio~ permit obtained fro~ this office, or <br />until earlier cancellation pursuant tc the Environmental Quality commission <br />rules.
