However, site conditions will require the installa-
tion of a STANDARD SYSTEH for the original syste~ and a SAND ~ILTER
future replacement system. These are described in the at~aehed ¢onstrustion
detail and setback sheets.

This approval is given on the basis that the lo~ or parcel will not be
further partitioned or subdivided and that conditions on this property and on
adjacent properties are not altered in any manne= which would prohibit the
issuance of a permit. Soils in the approved a~ea shall not be altered or
modified by cutting, filling, or other develol~nental activities which would
impede the successful operation of the initial or replacement soil absorption
systems. Failure to coa~ly with the above items may void this approval.

This report is not a De~f~ tO install the sewage svst~l. A Der~it must
be obtained before ~DV sv=tem construction occurs. A permit may be issued only
after the property has been found to be compatible with applicable LCDC
acknowledged local compr~hensiv~ land use plan. A Statement Of Land Use
compatibility Or its equivalent is required before a construction permit can
be issued.

A detailed plot plan, drawn to scale (ex. 1" = 60'), must be submitted
with the permit application before the construction-installation permit Gan be
issued. The system must be installed by the property owner or a DEQ licensed
septic installer.

This site evaluation ~epert is valid until an on-site sewage system is
installed l~ureuant to the construction pe~it obtained from this office, or
until earlier cancellation pursuant to the Environmental Quality commission
rules, subject to the abo~e, thi~ report runs with the land and will automati-
cally benefit subsequent owners.

Our office hours are ~o~day through ~riday, S~00 to 4=30.
