Marion County

OREGON
COMMUNITY DEVELOPMENT
DEPARTMENT

NOTICE OF APPROVAL:

EVALUATION REPORT FOR AN
ON-SIT~ S~WA~E DISPOSAL SYSTEM

February 6, 1~95

GLORIA JOHNSON
11485 JAMES WAY SE
AUMSVILL~, OREGON

97325

SUBJECT: SITE #94-4605,
SITE ADDRESS:

FiLE

T:dS, R:iW, S:29D, W FO T/L:500
11483 JAMES WAY SE. AUMSVILLE, OREGON 97325

Our office has completed the evaluation for the proposed installation of
an on-site sewage system. The septic system is proposed to serve a three
bedroom on the property.

~ased on the results of this study, on-site sewage disposal appears
feasible for the property. ~owever, site conditions will require the installa-
tion of a STANDARD SYSTEM for the original system and a SAND FILTER SYSTEM
for the future replacement system. These are described in the attached
construction detail and setback sheets.

This approval is given on the basis that the lot or parcel will not be
further partitioned or subdivided and that conditions on this property and on
adjacent properties are not altered in any markner which would prohibit the
issuance of a permit. Soils in the approved area shall not be altered or
modifie~ by cutting, filling, or other developmental activities which would
impede the successful operation of the initial or replacement soil absorption
systems. Failure to comply with the above items may void this approval.

This r~9ort is not a permit to install the sewage System. A Dermlt must
be obtained before any system construction occurs. A permit may be issued only
after the property has been found to be compatible with applicable LCDC
acknowledged local comprehensive land use plan. A Statement of Land Use
Compatibility or its equivalent is required before a construction permi$ can
be issued.

A detailed plot plan, drawn to scale (ex. 1" = 60'), must be submitted
with the permit application before the construction-installation permit can be
issued. The system must be installed by the property owner or a DEQ licensed
septic installer.

This site evaluation report is valid until an on-site sewage system is
installed pursuant to the construction permit obtained from this office, or
until earlier cancellation pursuant to the Environmental Quality Commission
rules, subject to the above, this report runs with the land and will automati-
cally benefit subsequent owners.

Our office hours are MOnday through Friday, 8:00 to 4:30. Flease contact
our office if you have any questions concerning this report.

LEE A, GRIMES, SOIL SCIENTIST
