Marion County

OREGON
COMMUNITY DEVELOPMENT
DI~PARTMENT

NOTICE OF APPROVAL:

EVALUATION REPORT FOR Alq
ON-SITE SEWAOE DISPOSAL SYSTEM

May 4, 1995

ALAN TURNER
C/O JAMES HANSEN
3340 COMMERCIAL STREET SE
SALEM, OREGON 97302

S~BJECT: SITE #94-4042,
SITE ADDRESS:

FILE

T:SS, R:2W, S:26C, PO T/L:iS00
7585 SUNNYBROOK LANE SE. AUMSVILLE, OR 97325

Our office has completed the evaluation for the proposed installation of
an on-site sewage system. The septic system is proposed to serve a three
bedroom residence on the property.

Based on the results of this study, this area does not appear suitable
for a standard on-site sewage system. However, the site appears suitable for
the installation of an alternative SD_ND FILTER PLUS FULL CAPPING FILL SYSTEMS
as described in the attached construction detail and setback sheets.

This approval is given on the basis that the lot or parcel will not be
further partitioned or subdivided and that conditions on this property and on
adjacent properties are not altered in any manner which would prohibit the
issuance of a permit. Soils in the approved area shall not be altered or
modified by cutting, filling, or other developmental activities which would
impede the successful operation of the initial or replacement soil absorption
systems. Failure to comply with the above items may void this approval.

This report is not a permit to install the sewaqe system. A permit must
be obtained before any system construction occurs. A permit may be issued only
after the property has been found to be compatible with applicable LCDC
acknowledged local comprehensive land use plan. A Statement of Land Use
Compatibility Or its ecI1/ivalent is reqllired before a construction permit can
be issued.

A detailed plot plan, drawn to scale (ex. 1,, = 60'), must be submitted
with the permit application before the construction-installation permit can be
issued. The system must be installed by the property owner or a DEQ licensed
septic installer,

This site evaluation report is valid until an on-site sewage system is
installed pursuant to the construction permit obtained from this office, or
until earlier cancellation pursuant to the Environmental Quality Commission
rules. Subject to the above, this report runs with the land and will automati-
cally benefit subsequent owners.

Our office hours are Monday through Friday, 8:00 to 4:30. Please contact
our office if you have any questions concerning this report.

LEE A. GRIMES, SOIL SCIENTIST
