NOTICE TO MORTGAGEE, LIENHOLDER, VENDOR, OR SELLER: ORS CHAPTER <br />215 REQUIRES THAT IF YOU RECEIVE THIS NOTICE, IT MUST PROMPTLY BE <br />FORWARDED TO THE PURCHASER. <br />NOTICE OF DECISION <br />PARTITIONING CASE NO. 03-19 <br />APPLICATION: Application of Billy Gostevskyh and Zinovei & Kalinika Shadrin to divide a <br />0.91 acre parcel into three parcels containing 19,685 square feet, 14,652 square feet, and 6,567 <br />square feet each in a RS (Single Family Residential) zone located at 4111 Hayesville Drive NE, <br />Salem. (T7S; R2W; Section 6CA; Tax Lot 2400) <br />DECISION: Notice is hereby given that the Planning Director for Marion County has <br />APPROVED the above described Partitioning application subject to certain conditions. <br />EXPIRATION DATE: This approval is valid only when the final partitioning plat is recorded <br />by AUGUST 12, 2005. After recording the final partitioning plat, the applicant shall provide <br />proof of the recording to the Planning Division. This can be either a copy of the recording fee <br />receipt or a copy of the plat showing the County Clerk's stamp. The effective period of an <br />approved application may be extended for an additional year subject to approval of an extension <br />(Extension form available from the Planning Division). Additional extensions may not be <br />granted if the regulations under which this decision was granted have changed since the <br />original approval. <br />WARNING: A decision approving the proposed division or use is for land use purposes only. <br />To be sure the subject property can accommodate the proposed use the applicant needs to check <br />with the Building Inspection Division, (503) 588-5147. <br />This decision does not include approval of a building permit. <br />CONDITIONS: The following conditions must be met before a building permit can be obtained <br />or the approved use established: <br />The applicant shall submit a survey per ORS 92.050, and a plat must be submitted for <br />review to the County Surveyor's Office (5155 Silverton Road NE, 503 588-5155). <br />2. The applicant shall submit a final partitioning plat to the County Surveyor's Office, along <br />with required fees and a Plant Service report and/or title report. Following plat approval, <br />it shall be recorded with the Marion County Clerk (final plat instructions and example <br />enclosed). <br />3. All access ways and easements serving the lots shall be named. The applicant shall <br />include the accepted street name "Tree Lane NE" on the partition plat.
