The conditions <br /> observed during this site evaluation indicate that the area around the test pit is <br /> capable of supporting a STANDARD system. Plans are approved as revised. Please <br /> refer to the signed approved site plan dated April 21, 2020. Any deviation from the <br /> approved plans must be authorized by this office prior to installation or the permit may <br /> be voided. It is the responsibility of applicant/installer to coordinate these <br /> requirements with all others in OAR 340-071-0220. <br /> This Permit is for a system designed to serve a single family dwelling with a maximum <br /> of 5 bedrooms. Peak sewage flow into the system is limited to a maximum of 525 <br /> gallons per day, with an average sewage flow of not more than 263 gallons per day. <br /> Premature failure of the treatment system may occur if either of these flow quantities <br /> is exceeded. If, for some reason, you expect your domestic household water use may <br /> exceed these flows, it is advisable to increase the size of the treatment system. <br /> Specific Requirements <br /> TANK RISER: The tank must be installed with watertight risers extending to the <br /> ground surface or above. The riser lid must be gasketed and securely fastened. <br /> AN EFFLUENT LIFT PUMP WILL BE REQUIRED to install the septic system disposal <br /> field in the approved area and at the required depth. Detailed instructions for dosing <br /> tank/effluent pump installations are available in our office. The electrical wiring to the <br /> pump and alarm must be installed in accordance with the appropriate Electrical Code. <br /> The electrical work must be done under a Marion County Electrical Permit and must be <br /> inspected and approved by a Marion County Electrical Inspector. The work must be <br /> left uncovered and accessible for inspection. The electrical inspection includes the <br /> connections to the pump and alarm in the dosing tank, the wiring in the trench and the <br /> connections in the structure. The effluent pipe must have 18-gauge minimum green <br /> tracer wire or green metallic tape placed along the line. <br /> HEADER PIPE between Distribution Box and drainfield trench must measure a <br /> minimum of 4 feet, must be installed level, and bedded on undisturbed earth. <br /> OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the <br /> Department of Environmental Quality rules.
