To: Michael Roantree <br /> From: Robert Goodwin, REHS <br /> RE: Major repair permit no. 555-21-008486 <br /> March 2, 2022 <br /> Page 2 <br /> Under the conditions observed at the site, a seepage bed system is required, and <br /> will be permitted as a reasonable repair per OAR 340-071-0215(4)(b) to allow a 50' <br /> setback to the existing well. <br /> Please see attached soil notes form and onsite septic inspection map for the <br /> approved area location and minimum requirements for the seepage bed system. A <br /> plot plan showing all required information including a materials list, line lengths, <br /> pump curve, and setback information must be submitted to this office for a permit <br /> to be issued. There is a second well on the property a few feet closer to the test pit, <br /> if a 50' setback cannot be maintained to this well please abandon it and submit <br /> documentation from the licensed well driller.
