Septic Site Evaluation 555-22-006542-EVAL Page 3 of 4 <br /> Date issued 08108/2022 <br /> • <br /> Apphcatton status Slte Evaluatton Approved r' <br /> Work descrl!ptton SE FUTURE„3 BDRMSFD(COMBINING 5 LOTS' II4T01�}� of <br /> Conditions of approval: <br /> -Approved Area&System Type <br /> •Conditions observed during this site evaluation indicate that the area around the evaluated test holes is not capable of <br /> supporting a standard septic system due to the following limitations: <br /> •Limited Space <br /> •Description: There is not sufficient space in the evaluated area for both the initial and repair drainfield areas meeting all <br /> required setbacks.The property is small(0.25 acres proposed)and fitting an initial and repair drainfield is impossible for a <br /> standard system. <br /> •Under the conditions observed at the test pits, an Advanced Treatment Technology(AU),Sand Filter,or <br /> Recirculating Gravel Filter(RGF)system is required. <br /> -Specific Requirements <br /> •Refer to the Soil Notes Form and Onsite Septic Inspection Map which shows the approved areas,specific requirements and <br /> other details of the site visit. <br /> • <br /> •The owner of an ATT, Pressurized Distribution System or Sand Filter must maintain a service contract with a licensed and <br /> certified professional to service and maintain the onsite system. A service contract must be entered into before a certificate of <br /> satisfactory completion is issued and must be maintained until the system is-decommissioned. <br /> •AN EFFLUENT LIFT PUMP WILL BE REQUIRED to install the septic system disposal field in the approved area and at the <br /> required depth. Detailed instructions for dosing tank/effluent pump installations are available in our office.The electrical wiring <br /> to the pump and alarm must be installed in accordance with the appropriate Electrical Code.The electrical work must be done <br /> under a Marion County Electrical Permit and must be inspected and approved by a Marion County Electrical Inspector.The <br /> work must be left uncovered and accessible for inspection.The electrical inspection includes the connections to the pump and <br /> alarm in the dosing tank,the wiring in the trench and the connections in the structure. <br /> • <br /> •Additional Conditions of Site Approval ' <br /> •1. This site is approved for the type of disposal system described above. Peak sewage flow into the system is limited to a <br /> maximum of 450 gallons per day,with an average sewage flow of not more than 225 gallons per day. This is normally <br /> sufficient to serve a residence with 4 bedrooms. Premature failure of the treatment system may occur if either of these flow <br /> quantities is exceeded. If,for some reason,you expect your water use may exceed these flows, it may be advisable to <br /> increase the size of the treatment system. <br /> •2. Any alteration of natural soil conditions(i.e.cutting or filling)in the acceptable area may void this approval. <br /> •3. Both the initial and replacement disposal areas are to be protected from traffic and livestock <br /> •4. The area must not be subjected to excessive saturation due to, but not limited to,artificial drainage of ground surfaces, <br /> roads,driveways and building down spouts. <br /> •5. This approval is given on the basis that the parcel described above will not be further partitioned or subdivided. <br /> •6. Placement of a well within 100 feet of the approved areas may invalidate this approval. <br /> •This site approval is valid until the system approved above is constructed in accordance with a Marion County issued <br /> Construction-Installation permit. Technical rule changes shall not invalidate this approval, but may require use of a different <br /> kind of system. The site approval runs with the land and will automatically benefit subsequent owners. <br /> • <br /> •The purpose of the evaluation is to locate suitable soils in an area that is large enough for both the initial drainfield area and <br /> CALL BEFORE YOU DIG...ITS THE LAW <br /> ATTENTION:Oregon law requires you to follow rules adopted by the Oregon Utility Notification Center.
