Mr. Jason Caswell ProNet File No. 47371
State Farm Insurance Company March 15, 2021
Page 7
• Serial No.: NF15114386
• Electrical Ratings: 120-volt, 10-amp, single-phase, 60-hertz
• Year of Manufacture: 2011
We did not observe any evidence of damage on this appliance, but note that during
operation attempts, the range malfunctioned whenever we attempted the convection
mode of operation since the convection fan would never operate. The warming drawer
also did not operate. There were also other abnormalities present on the front digital
display screen.
Dishwasher: We inspected the dishwasher located in the kitchen on the east side of
the main residence. Major specifications for the range were as follows:
• Manufacturer/Distributer: KitchenAid
• Model No.: KUDS30IVBL3
• Serial No.: FY4607995
• Electrical Ratings: 120-volt, single-phase, 60-hertz
• Year of Manufacture: 2014
We did not observe any evidence of damage on this appliance, but note that during
operation attempts, the dishwasher malfunctioned as it would never advance the
current cycle of operation. Also, whenever we attempted to operate the "CANCEL"
button, the button would erroneously function as an on/off switch.
Shop House (Garage).
General: We did not observe evidence of fire-related damage around any of the
interior shop house areas.
Sub-panel: We inspected the sub-panel which was surface mounted between wall
studs on the east wall inside the shop house. We also removed the front panel barrier
as part of our inspection to examine the internal cables, components, and neutral and
ground wiring connections. All breakers were in the"ON" position. We did not observe
any evidence of damage, inappropriate wiring/connections, or other abnormalities.
