Inspections were done, 8/10/16, by Matt Puntney approving the <br /> INSPECTION tank install per OAR 340-071; however, a signed asbuilt document and a <br /> tank abandonment form with a pump receipt were never submitted to this <br /> EMERGENCY office. An asbuilt document of licensed and certified septic installer is <br /> MANAGEMENT required before a Certificate of Satisfactory Completion is issued by this <br /> ENGINEERING office. <br /> ENVIRONMENTAL Per OAR 340-071-0175(6), A person may not connect to or use any <br /> SERVICES system completed after January 1, 1974, unless a Certificate of <br /> Satisfactory Completion has been issued for the installation or deemed <br /> OPERATIONS issued by operation of law in accordance with this rule. <br /> PARKS Per OAR 340-071-160(5), Permit effective dates, A permit issued for <br /> PLANNING construction of a system under this rule is effective for one year from the <br /> date of issuance.
