SE, Turner Oregon <br /> DIRECTOR <br /> Brian Nicholas,P.E. Dear Ms. Woodward <br /> ADMINISTRATION <br /> On November 17, 2017, a septic installation permit 555-17-008852 was <br /> BUILDING issued for the installation of a septic system located at the above <br /> INSPECTION mentioned property. Inspections were done, 12/28/17 and 01/02/18, by <br /> Matt Puntney, approving the system install per OAR 340-071; however, a <br /> EMERGENCY signed asbuilt document, with a map of the installed system as well as a <br /> MANAGEMENT materials list and a tank abandonment form with a pump receipt were <br /> ENGINEERING never submitted to this office. An asbuilt document signed by a licensed <br /> and certified septic installer and a tank abandonment form are both <br /> ENVIRONMENTAL required before a Certificate of Satisfactory Completion is issued by this <br /> SERVICES office. <br /> OPERATIONS Per OAR 340-071-0175(6), A person may not connect to or use any <br /> PARKS system completed after January 1, 1974, unless a Certificate of <br /> Satisfactory Completion has been issued for the installation or deemed <br /> PLANNING issued by operation of law in accordance with this rule. <br /> SURVEY Per OAR 340-071-160(5), Permit effective dates, A permit issued for <br /> construction of a system under this rule is effective for one year from the <br /> date of issuance.
