Dear Mr. Elwood, <br /> ADMINISTRATION <br /> On September 20, 2017, septic installation permit 555-17-007983 was <br /> BUILDING issued to David J. Kammerman for the repair installation of a septic tank <br /> INSPECTION and drainfield located at the above mentioned address. An inspection was <br /> done on 11/03/17, by Matt Puntney and the drainfield installation was <br /> EMERGENCY approved. However, a signed asbuilt document was never submitted to , <br /> MANAGEMENT this office. An asbuilt document signed by the licensed and certified septic <br /> ENGINEERING installer that has installed the system or the homeowner that has installed <br /> the septic system is required before a Certificate of Satisfactory <br /> ENVIRONMENTAL Completion can be issued by this office. A signed tank abandonment form <br /> SERVICES with a tank pump receipt is also required as well. <br /> OPERATIONS Per OAR 340-071-0175(6), A person may not connect to or use any <br /> PARKS system completed after January 1, 1974, unless a Certificate of <br /> Satisfactory Completion has been issued for the installation or deemed <br /> PLANNING issued by operation of law in accordance with this rule. <br /> SURVEY Per OAR 340-071-160(5), Permit effective dates, A permit issued for <br /> construction of a system under this rule is effective for one year from the <br /> date of issuance.
