The conditions observed during this site evaluation <br /> indicate that the area around the test pit is capable of supporting a STANDARD system for the REPAIR. Plans are approved <br /> as revised. Please refer to the signed approved site plan dated September 19,2017. Any deviation from the approved plans <br /> must be authorized by this office prior to installation or the permit may be voided. It is the responsibility of applicant/installer to <br /> coordinate these requirements with all others in OAR 340-071-0220. <br /> This Permit is for a system designed to serve a single family dwelling with a maximum of 4 bedrooms. Peak sewage flow into <br /> the system is limited to a maximum of 450 gallons per day,with an average sewage flow of not more than 225 gallons per <br /> day. Premature failure of the treatment system may occur if either of these flow quantities is exceeded. If,for some reason, <br /> you expect your domestic household water use may exceed these flows,it is advisable to increase the size of the treatment <br /> system. <br /> Specific Requirements <br /> EFFLUENT SEWER:The effluent sewer is the pipe from the septic tank to the distribution box. The material required is 3034 <br /> or Schedule 40 pipe with 18-gauge minimum green tracer wire or green metallic tape placed along the pipe. The effluent <br /> sewer shall extend at least five(5)feet from the tank. There must be a minimum difference of eight(8)inches between the <br /> invert(flow line)of the septic tank outlet and the invert of the distribution or drop box outlet. In Equal Distribution,there must <br /> be six(6)inches of fall between the septic tank outlet and the distribution box inlet. In Serial Distribution,there must be two(2) <br /> inches of fall between the septic tank outlet and the drop box inlet. <br /> DRY SOIL CONDITIONS ARE REQUIRED FOR INSTALLATION(March 1—October 31) The geographical area in which the <br /> property lies is considered a risk to installation during winter months. Soil moisture conditions generally preclude construction <br /> of the drainfield in the winter months. <br /> THE ATTACHED SEPTIC TANK ABANDONMENT FORMMUST BE COMPLETED AND RETURNED TO OUR OFFICE. The <br /> septic system installer must complete and sign the form and attach a copy of the receipt from the septic tank pumping service <br /> for pumping the tank. <br /> HEADER PIPE between Distribution Box and drainfield trench must measure a minimum of 4 feet,must be installed level, and <br /> bedded on undisturbed earth. <br /> OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the Department of Environmental Quality rules. <br /> A copy of the DEQ setback requirements is available in our office and online at <br /> http.//www.oregon.gov/deq/Residential/Pages/Onsite.aspx under Rules: Division 71:Table 1. <br /> THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, livestock and farming <br /> activities. Other activities such as excavating or filling,driveway placement, paving or other similar activities that alter the <br /> natural soil conditions will adversely affect the soil and the septic system function and lead to premature system failure. The <br /> area must not be subjected to excessive saturation due to artificial drainage of ground surfaces,driveways, building <br /> downspouts or other sources. The areas of the initial septic system and the future repair system must both be protected. <br /> MAINTAIN PERMANENT ACCESS to septic tank for pumping and service.The septic tank should be pumped and inspected <br /> by a licensed septic tank pumping service before sludge and scum accumulations reach 30%of the tank liquid operating <br /> volume. Pumping every 3 to 5 years is an average interval. The septic tank is a reservoir for the solid materials in the sewage <br /> waste stream. Excessive accumulation of these solid materials in the septic tank will cause the solids to wash out of the tank <br /> and into the drainfield.
