An inspection was done on <br /> INSPECTION 12/13/17, by Matt Puntney and the septic tank installation was partially <br /> approved, a pump, alarm and watertight test still needs to be performed as <br /> EMERGENCY well as the electrical needs to inspected by a Marion County Septic <br /> MANAGEMENT Inspector. In addition a signed asbuilt document was never submitted to <br /> ENGINEERING this office. An asbuilt document signed by the licensed and certified septic <br /> installer that has installed the system or the homeowner that has installed <br /> ENVIRONMENTAL the septic system is required before a Certificate of Satisfactory <br /> SERVICES Completion can be issued by this office. A signed tank abandonment form <br /> with a tank pump receipt is also required as well. <br /> OPERATIONS <br /> PARKS Per OAR 340-071-0175(6), A person may not connect to or use any <br /> system completed after January 1, 1974, unless a Certificate of <br /> PLANNING Satisfactory Completion has been issued for the installation or deemed <br /> issued by operation of law in accordance with this rule. <br /> SURVEY <br /> Per OAR 340-071-160(5), Permit effective dates, A permit issued for <br /> construction of a system under this rule is effective for one year from the <br /> date of issuance. This permit has now expired. <br /> In order to issue a Certification of Satisfactory Completion for this system, <br /> a new permit application must be submitted with associated fees and <br /> documentation, a signed asbuilt document and drawing will also need to <br /> be submitted and a site visit and inspection may be required if deemed to <br /> be necessary in order to final the installation. <br /> 5155 Silverton Road NE•Salem,OR 97305-3802• www.co.marion.or.us/PW <br /> Printed on recycled paper• Reduce—Reuse—Recycle-Recover <br />
