Dear Amanda <br /> ADMINISTRATION <br /> On August 17, 2017, septic installation permit 555-17-005317 was issued <br /> BUILDING for the installation of a septic tank and drainfield located at the above <br /> INSPECTION mentioned address. Inspections were done on 012/8/17 and 6/26/18, by <br /> Matt Puntney and the septic system installation was approved. However, a <br /> EMERGENCY signed asbuilt document with a materials list and map of the approved <br /> MANAGEMENT installation was never submitted to this office. An asbuilt document signed <br /> ENGINEERING by the licensed and certified septic installer that has installed the system or <br /> the homeowner that has installed the septic system is required before a <br /> ENVIRONMENTAL Certificate of Satisfactory Completion can be issued by this office. <br /> SERVICES <br /> Per OAR 340-071-0175(6), A person may not connect to or use any <br /> OPERATIONS system completed after January 1, 1974, unless a Certificate of <br /> PARKS Satisfactory Completion has been issued for the installation or deemed <br /> issued by operation of law in accordance with this rule. <br /> PLANNING <br /> Per OAR 340-071-160(5), Permit effective dates, A permit issued for <br /> SURVEY construction of a system under this rule is effective for one year from the <br /> date of issuance. This permit has now expired. <br /> In order to issue a Certification of Satisfactory Completion for this system, <br /> a new permit application must be submitted with associated fees and <br /> documentation, a signed asbuilt document and drawing will also need to <br /> be submitted and a site visit and inspection may be required if deemed to <br /> be necessary in order to final the installation. <br /> 5155 Silverton Road NE•Salem,OR 97305-3802• www.co.marion.or.us/PW <br /> Printed on recycled paper• Reduce—Reuse—Recycle-Recover <br />
