A 4-ttz'.n. <br /> Conditions of Approval <br /> • <br /> This property has been approved for a STANDARD SEPTIC SYSTEM in the area identified in Site <br /> Evaluation Report 555-17-001718-EVAL. System plans are approved as submitted. Refer to approved plans <br /> dated July 19, 2017. Any deviation from the approved plans must be authorized by this office prior to <br /> installation or the permit may be voided. It's the responsibility of applicant/installer to coordinate these <br /> requirements with all others in OAR 340-071-0220. <br /> • <br /> This permit is for a system designed to serve a commercial facility with up to 20 employees. Peak <br /> sewage flow into the system is limited to a maximum of 300 gallons per day, with an average sewage flow of <br /> not more than 150 gallons per day. Premature failure of the treatment system may occur if either of these <br /> flow quantities is exceeded. If, for some reason, you expect your water use may exceed these flows, it is <br /> advisable to increase the size of the treatment system. <br /> • <br /> AN EFFLUENT LIFT PUMP WILL BE REQUIRED to install the septic system disposal field in the <br /> approved area and at the required depth. Detailed instructions for dosing tank/effluent pump installations are <br /> available in our office. The electrical wiring to the pump and alarm must be installed in accordance with the <br /> appropriate Electrical Code. The electrical work must be done under a Marion County Electrical Permit and <br /> must be inspected and approved by a Marion County Electrical Inspector. The work must be left uncovered <br /> and accessible for inspection. The electrical inspection includes the connections to the pump and alarm in the <br /> dosing tank, the wiring in the trench and the connections in the structure. The effluent pipe must have <br /> 18-gauge minimum green tracer wire or green metallic tape placed along the line. <br /> • HEADER PIPE between Distribution Box and drainfield trench must measure a minimum of 4 feet, <br /> must be installed level, and bedded on undisturbed earth. <br /> TANK RISER: The tank must be installed with watertight risers extending to the ground surface or <br /> above. The riser lid must be gasketed and securely fastened. <br /> OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the DEQ rules. A copy of <br /> the DEQ setback requirements is available online at <br /> http://www.oregon.gov/deq/Residential/Pages/Onsite.aspx under Rules: Division 71: Table 1 and in our <br /> office. <br /> • MAINTAIN PERMANENT ACCESS to septic tank for pumping and service.The septic tank should be <br /> pumped and inspected by a licensed septic tank pumping service before sludge and scum accumulations reach <br /> 30% of the tank liquid operating volume. Pumping every 3 to 5 years is an average interval. The septic tank <br /> is a reservoir for the solid materials in the sewage waste stream. Excessive accumulation of these solid <br /> materials in the septic tank will cause the solids to wash out of the tank and into the drainfield. This will <br /> cause irreversible damage to the drainfield and will eventually lead to a system failure. <br /> • THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, livestock <br /> and farming activities. Other activities such as excavating or filling, driveway placement, paving or other <br /> similar activities that alter the natural soil conditions will adversely affect the soil and the septic system <br /> function and lead to premature system failure. The area must not be subjected to excessive saturation due to <br /> artificial drainage of ground surfaces, driveways, building downspouts or other sources. The areas of the <br /> initial septic system and the future repair system must both be protected. <br /> • THIS SYSTEM MUST BE INSPECTED AND APPROVED PRIOR TO BACKFILL. When work is <br /> complete and ready for inspection, request an inspection by following the instructions on the accompanying <br /> permit document. Be sure the System Record form with the As-Built drawing is on the job or in our office <br /> when the inspection is requested.
