Based on the results of this study, site evaluation approval is granted. The approval is site specific to the area tested, and the septic system must be located in the immediate area of the test pits, unless otherwise authorized by this office. A new site evaluation may be required if thesystem is proposed for a different area on the property. This approval is given on the basis that the lot or parcel will not be further partitioned o subdivided and that conditionson this property and onadjacent properties arenot altered inany mannerwhich would prohibit the issuanceof a permit. Soils in th approved areashall not be altered ormodifiedbycutting, filling, or othersuccessful activities which would impede the s ssful operation of the initial or replacement soil absorption systems. Failure to comply with the above items may void this approval. This report is not a permit to install the sewage system. A permit must be obtained before any system construction occurs. A permit may be issued only after the property has been found to be compatible with applicable LCDCacknowledged local comprehensive land use e plan. A "Statement of Land Use Compatibility" or its equivalent is required before a construction permit canbe issued. A detailed site plan, drawn to scale (ex: 1" = 60"), must be submitted with the permit application before the cnstruction-installation permit can be issued. The system must be installed by the property owner or a DEO licensed installer. This site evaluation report is valid until an on-site sewage system is installed pursuant to the c nstruction permit obtained from this office, oruntil cancelation pursuant to the Environmental Quality Commission Rules. Subject to the above, this report runs with the land and will automatically benefit subsequent owners. The trenches are perforated pipe and are installed level (+/- one inch) and contoured to follow the natural ground surface.
