Based on the
results of this study, site evaluation approval is granted. The approval is site specific to the area tested,
and the septic system must be located in the immediate area
of the test pits, unless otherwise authorized by
this office. A new site evaluation may be required if thesystem is proposed for a different area on the
property.
This approval is given on the basis that the lot or parcel will not be further partitioned o subdivided and
that conditionson this property and onadjacent properties arenot altered inany mannerwhich would prohibit
the issuanceof a permit. Failure to comply with the above items may void this approval.
This report is not a permit to install the sewage system. A permit must be obtained before any system
construction occurs. A permit may be issued only after the property has been found to be compatible with
applicable LCDCacknowledged local comprehensive land use
e plan. A "Statement of Land Use Compatibility" or its
equivalent is required before a construction permit canbe issued.
A detailed site plan, drawn to scale (ex: 1" = 60"), must be submitted with the permit application before the
cnstruction-installation permit can be issued. The system must be installed by the property owner or a DEO
licensed installer.
This site evaluation report is valid until an on-site sewage system is installed pursuant to the c nstruction
permit obtained from this office, oruntil cancelation pursuant to the Environmental Quality Commission Rules.
Subject to the above, this report runs with the land and will automatically benefit subsequent owners.
Site conditions will require the following installation specifications:
TYPE OF INITIAL SYSTEM: ALTR
TYPE OF FUTURE REPLACEMENT SYSTEM: ALTR
MINIMUM SEPTIC TANK CAPACITY: 1000 GALLONS
PUMP TANK CAPACTIY: 500
LINEAL FEET OF DRAINFIELD: 150
TYPE OF DISTRIBUTION:
DISPOSAL TRENCH DEPTH: 12 INCHES,MINIMUM; 12 INCHES,MAXIMUM
TOTAL DRAIN MEDIA DEPTH: 12
DRAIN MEDIA UNDER PIPE: 6
MINIMUM SOIL BACKFILL: 16
GROUND WATER INTERCEPTOR:
TOTAL if PROPOSED BEDROOMS: 3
TOTAL k PROPOSED EMPLOYEES: 0
TOTAL 4 SEATING: 0
The disposal trenches are nstructed two (2) feet wide and on um ten (10) foot centers.
