The conditions observed <br /> during this site evaluation indicate that the area around the test pits is capable of supporting a <br /> STANDARD system for the ALTERATION. Plans area roved as REVISED. Please refer to the <br /> Y PP <br /> signed approved site plan dated January 18, 2019. <br /> Any deviation from the approved plans must be authorized by this officeP rior to installation or the <br /> permit may be voided. It is the responsibility of applicant/installer to coordinate these requirements <br /> with all others in OAR 340-071-0220. <br /> This Permit is for a system designed to serve a single family dwelling with a maximum of 4 <br /> bedrooms. Peak sewage flow into the system is limited to a maximum of 450 gallons per day, with <br /> an average sewage flow of not more than 225 gallons per day. Premature failure of the treatment <br /> system may occur if either of these flow quantities is exceeded. If, for some reason, you expect <br /> your domestic household water use may exceed these flows, it is advisable to increase the size of <br /> the treatment system. <br /> Specific Requirements <br /> TANK RISER: A watertight riser must be installed to extend to the ground surface or above. The <br /> riser lid must be gasketed and securely fastened. <br /> EFFLUENT SEWER:The effluent sewer is the pipe from the septic tank to the distribution box. <br /> The material required is 3034 or Schedule 40 pipe with 18-gauge minimum green tracer wire or <br /> green metallic tape placed along the pipe. The effluent sewer shall extend at least five (5)feet <br /> from the tank. There must be a minimum difference of eight(8) inches between the invert(flow <br /> line) of the septic tank outlet and the invert of the distribution or drop box outlet. In Equal <br /> Distribution, there must be six(6) inches of fall between the septic tank outlet and the distribution <br /> box inlet. In Serial Distribution, there must be two(2) inches of fall between the septic tank outlet <br /> and the drop box inlet. <br /> HEADER PIPE between Distribution Box and drainfield trench must measure a minimum of 4 feet, <br /> must be installed level, and bedded on undisturbed earth. <br /> DO NOT INSTALL SYSTEM DURING WINTER MONTHS (Nov. 1 —March 1) <br /> The geographical area in which the property lies is considered a risk to installation during winter <br /> months. Therefore, for this installation a Marion County Onsite Wastewater Specialist must <br /> specifically approve soil moisture conditions at this site for installation to occur during the period <br /> from November 1st to March 1st. This mayinclude a field visit directly prior to installation. <br /> OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the Department of <br /> Environmental Quality rules.
