Septic Permit 555-19-003668-PRMT Page 3 of 4 <br /> Date Cert�f"icate Issued 06/03/2019 a' - � `'. - �' <br /> Work Description MINOR REPAIR TANK ONLY" <br /> The septic tank installation plan is approved as submitted. Please refer to the signed <br /> approved site plan dated May 21, 2019. Any deviation from the approved plans must be <br /> authorized by this office prior to installation or the permit may be voided. <br /> TANK RISER: The tank must be installed with watertight risers extending to the ground <br /> surface or above. The riser lid must be gasketed and securely fastened. <br /> Tanks must be protected from vehicular traffic at all times. Permanent barricades <br /> are required. <br /> EFFLUENT SEWER: The effluent sewer is the pipe from the septic tank to the distribution <br /> box. The material required is 3034 or Schedule 40 pipe with 18-gauge minimum green <br /> tracer wire or green metallic tape placed along the pipe. The effluent sewer shall extend at <br /> least five (5) feet from the tank. There must be a minimum difference of eight (8) inches <br /> between the invert (flow line) of the septic tank outlet and the invert of the distribution or <br /> drop box outlet. In Equal Distribution, there must be six (6) inches of fall between the <br /> septic tank outlet and the distribution box inlet. <br /> OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the <br /> Department of Environmental Quality rules. A copy of the DEQ setback requirements is <br /> available in our office and online at <br /> http://www.oregon.gov/deq/Residential/Pages/Onsite.aspx under Rules: Division 71: Table <br /> 1. Where the system crosses a property line, and easement has been recorded Reel 4194 <br /> Page 298, Marion County Clerk's Office. <br /> THE ATTACHED SEPTIC TANK ABANDONMENT FORM MUST BE COMPLETED AND <br /> RETURNED TO OUR OFFICE. The septic system installer must complete and sign the <br /> form and attach a copy of the receipt from the septic tank pumping service for pumping the <br /> tank. <br /> MAINTAIN PERMANENT ACCESSto septic tank for pumping and service. The septic <br /> tank should be pumped and inspected by a licensed septic tank service before <br /> sludge and scum accumulations reach 30% of the tank liquid operating volume. <br /> Pumping every 3 to 5 years is an average interval. The septic tank is a reservoir for the <br /> solid materials in the sewage waste stream. Excessive accumulation of these solid <br /> materials in the septic tank will cause the solids to wash out of the tank and into the <br /> drainfield.
