Septic Permit 555-18-008647-PRMT Page 3 of 4 <br /> Date Certificate Issued 06/11/2019.. , <br /> Work Description NEW INSTALL=4 BDRM <br /> Approved Area & System Type: <br /> This property has been approved for a STANDARD SEPTIC SYSTEM in the area identified <br /> for Lot 1 of Randall's Elkhorn Third Addition in Site Evaluation Report 80-784. System plans are <br /> approved as revised. Refer to approved plans dated February 7, 2019. Any deviation from <br /> the approved plans must be authorized by this office prior to installation or the permit may <br /> be voided.r It's the responsibility of applicant/installer to coordinate these requirements with <br /> all others in OAR 340-071-0220. <br /> This permit is for a system designed to serve a single family dwelling with a maximum of 4 <br /> bedrooms. Peak sewage flow into the system is limited to a maximum of 450 gallons per <br /> day, with an average sewage flow of not more than 225 gallons per day. Premature failure <br /> of the treatment system may occur if either of these flow quantities is exceeded. If, for <br /> some reason, you expect your domestic household water use may exceed these flows, it is <br /> advisable to increase the size of the treatment system. <br /> Specific Requirements: <br /> EFFLUENT SEWER:The effluent sewer is the pipe from the septic tank to the distribution <br /> box. The material required is 3034 or Schedule 40 pipe with 18-gauge minimum green <br /> tracer wire or green metallic tape placed along the pipe. The effluent sewer shall extend at <br /> least five (5) feet from the tank. There must be a minimum difference of eight (8) inches <br /> between the invert (flow line) of the septic tank outlet and the invert of the drop box outlet. <br /> In Serial Distribution there must be two (2) inches of fall between the septic tank outlet and <br /> the drop box inlet. <br /> HEADER PIPE between Distribution Box and drainfield trench must measure a minimum of <br /> 4 feet, must be installed level, and bedded on undisturbed earth. <br /> TANK RISER: The tank must be installed with watertight risers extending to the ground <br /> surface or above. The riser lid must be gasketed and securely fastened. <br /> OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the DEQ <br /> rules. A copy of the DEQ setback requirements is available in our office and online at <br /> http://www.oregon.gov/deq/Residential/Pages/Onsite-Resources.aspx and scroll to Table <br /> 1, "Minimum Separation Distances." <br /> MAINTAIN PERMANENT ACCESS to septic tank for pumping and service. The septic <br /> tank should be pumped and inspected by a licensed septic tank pumping service <br /> before sludge and scum accumulations reach 30% of the tank liquid operating <br /> volume. Pumping every 3 to 5 years is an average interval. The septic tank is a <br /> reservoir for the solid materials in the sewage waste stream.
