Septic Permit 555-19-000561-PRMT Page 3 of 5 <br /> Datek�C cate Issue 1E2/2019 ,°NO:c <br /> Work Description MA REPL SANOFIL WITH A UN T 0, DFM SFC s 3 <br /> � <br /> Existing System Information: <br /> Marion County File Records show a Certificate of Satisfactory Completion for a Sand Filter <br /> Septic System installed in 2001 under Permit No. 00-07419. It consists of a 1500-gallon <br /> two-compartment combo tank, a Sand Filter treatment unit and 177 lineal feet of drainfield <br /> line. The sand filter is failing and must be replaced. The other components will remain in <br /> use. <br /> This approval is for a system designed to serve a single family dwelling with a maximum of <br /> four bedrooms. Peak sewage flow into the system is limited to a maximum of 450 gallons <br /> per day, with an average sewage flow of not more than 225 gallons per day. <br /> The plan for installation of the treatment unit is approved as REVISED. Please refer to the <br /> signed approved site plan dated February 5, 2019. Any deviation from the approved plans <br /> must be authorized by this office prior to installation or the permit may be voided. It is the <br /> responsibility of applicant/installer to coordinate these requirements with all others in OAR <br /> 340-071-0220. <br /> SPECIFIC REQUIREMENTS <br /> ATT Pretreatment Unit: MicroFAST 0.5 as per approved attached plans. <br /> SERVICE CONTRACTS: The owner of an ATT system must maintain a service contract <br /> with a maintenance provider certified by the manufacturer to serve and maintain the onsite <br /> system. A service contract must be entered into before the system is installed and must <br /> be maintained until the system is decommissioned. The service contract must provide for <br /> a minimum of four inspections and service visits by a maintenance provider scheduled <br /> once every six months over a two-year period to inspect, adjust, and service the ATT. The <br /> septic tank must be pumped before solids reach 30% of the tank liquid operating <br /> level. The maintenance provider must submit an annual report and fee. Please review <br /> OAR 340-071-0345 for full requirements. The existing system has a service contract and <br /> current service record reports are on file. <br /> TANK RISER: The tank must be installed with watertight risers extending to the ground <br /> surface or above. The riser lid must be gasketed and securely fastened. <br /> ELECTRICAL COMPONENTSWILL BE REQUIRED for the ATT Unit. The electrical wiring <br /> to the pump and alarm must be installed in accordance with the appropriate Electrical <br /> Code. Relocation and alteration of electrical components must be done under a <br /> Marion County Electrical Permit and must be inspected and approved by a Marion <br /> County Electrical Inspector.
