Performance of Basic Services The Service Provider,shall perform the following Basic Services during the term of this Agreement: 1.1. Two(2)System Monitoring Inspections/Service Visits annually during each 12-month period after the contract Is signed. The Service Provider shall be responsible for submitting an annual report and annual evaluation fee to the appropriate regulatory agency•as requited iri OAR-071- 0345. During the term of this Agreement,Service Provider will provide a.total of four(4) monitoring/inspection visits and two(2)annual•repurts for submission tothe regulatory agency. - 1.2. The scope of the Basic Services Includes those described in Sections 3.0 and 4:0. Basic Services shall be performed during normal business hours Monday through Friday (excluding Federal holidays)on a pre-scheduled basis and as the Service•Provider•deems necessary or advisable.Emergency Service will be provided as stated below. At each service visit the system shall be inspected and serviced in accordance with the instruetiions in the System's O&M Manual.Additionally,an effluent quality inspection consisting of a visual assessment of dose,turbidity,and scum overflow and an olfactory assessment for odor shall be performed. The Service Provider will affix a"For Service,Call___. "label near the control panel's alarm signal and provide the contact person's name and phonenurfiber. Performance of the 1-year inspection/service visits shall include notification of needed repair, replacement,or additional paits used in the System. • The Service Provider shall provide emergency.service:within 24 hours of a:servicarequest. The Service Provider shall notify the Customer in writing if any Improper System operation cannot be remedied at the time of servicing. 2. Term of Agreement This Agreement shall he for the period'of 24 months from the date the contract is signed,unless otherwise terminated or canceled by either party as provided herein. 3. Definitions For purposes of the Agreement,the following definitions shall apply: 2 Year Septic Service Contract—Page 2
