The conditions observed during this site evaluation indicate <br /> that the area around the test pit is capable of supporting a STANDARD system for the REPAIR. Plans are approved as revised. <br /> Please refer to the signed approved site plan dated April 22,2019. Any deviation from the approved plans must be authorized <br /> by this office prior to installation or the permit may be voided. It is the responsibility of applicant/installer to coordinate these <br /> requirements with all others in OAR 340-071-0220. <br /> This Permit is for a system designed to serve a single family dwelling with a maximum of 4 bedrooms. Peak sewage flow into <br /> the system is limited to a maximum of 450 gallons per day,with an average sewage flow of not more than 225 per gallons per <br /> day. Premature failure of the treatment system may occur if either of these flow quantities is exceeded. ADDITIONAL <br /> DRAINFIELD IS HIGHLY RECOMMENDED DUE TO THE SITE OF THE FUTURE HARDSHIP DWELLING. If,for some <br /> reason,you expect your domestic household water use may exceed these flows, it is advisable to increase the size of the <br /> treatment system. <br /> Specific Requirements <br /> TANK RISER: The tank must be installed with watertight risers extending to the ground surface or above. The riser lid must be <br /> gasketed and securely fastened. <br /> HEADER PIPE between Distribution Box and drainfield trench must measure a minimum of 4 feet, must be installed level, and <br /> bedded on undisturbed earth. <br /> OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the Department of Environmental Quality rules. A <br /> copy of the DEQ setback requirements is available in our office and online at <br /> http://www.oregon.gov/deq/Residential/Pages/Onsite.aspx under Rules: Division 71:Table 1. <br /> THE ATTACHED SEPTIC TANK ABANDONMENT FORMMUST BE COMPLETED AND RETURNED TO OUR OFFICE. The <br /> septic system installer must complete and sign the form and attach a copy of the receipt from the septic tank pumping service <br /> for pumping the tank. <br /> DO NOT INSTALL SYSTEM DURING WINTER MONTHS(Nov.1—March 1) <br /> The geographical area in which the property lies is considered a risk to installation during winter months.Therefore,for this <br /> installation a Marion County Onsite Wastewater Specialist must specifically approve this site for installation from November 1st <br /> to March 1st. This may include a field visit directly prior to installation <br /> THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, livestock and farming activities. <br /> Other activities such as excavating or filling,driveway placement, paving or other similar activities that alter the natural soil <br /> conditions will adversely affect the soil and the septic system function and lead to premature system failure.'The area must not <br /> be subjected to excessive saturation due to artificial drainage of ground surfaces,driveways, building downspouts or other <br /> sources. The areas of the initial septic system and the future repair system must both be protected. <br /> MAINTAIN PERMANENT ACCESS to septic tank for pumping and service.The septic tank should be pumped and inspected by <br /> a licensed septic tank pumping service before sludge and scum accumulations reach 30%of the tank liquid operating volume. <br /> Pumping every 3 to 5 years is an average interval. The septic tank is a reservoir for the solid materials in the sewage waste <br /> stream. Excessive accumulation of these solid materials in the septic tank will cause the solids to wash out of the tank and into <br /> the drainfield.
