System plans are approved as submitted. Refer <br /> to approved plans dated September 6, 2018. The installation must conform to these approved plans. Any <br /> changes or product substitutions must be approved by this office prior to installation. Any substantial changes <br /> may require complete revised plans with a plan review fee. Failure to comply may invalidate construction <br /> approval and permit. The required inspection schedule is attached to this permit. It's the responsibility of <br /> applicant/installer to coordinate these requirements with all others in OAR 340-071-0220. <br /> This approval is for a system designed to serve a single family dwelling with a maximum of 4 bedrooms. Peak <br /> sewage flow into the system is limited to a maximum of 450 gallons per day,with an average sewage flow of <br /> not more than 225 gallons per day. Premature failure of the treatment system ma occur if either of these flow <br /> h <br /> quantities is exceeded. If,for some reason,you expect your domestic household water use may exceed these <br /> flows, it is advisable to increase the size of the treatment system. <br /> Specific Requirements <br /> ATT Pretreatment Unit: Advantex AX20 RT as per approved attached plan. <br /> SERVICE CONTRACTS: The owner of an ATT system must maintain a service contract with a maintenance <br /> provider certified by the manufacturer to serve and maintain the onsite system. A service contract must be <br /> entered into before the system is installed and must be maintained until the system is decommissioned. The <br /> service contract must provide for a minimum of four inspections and service visits by a maintenance provider <br /> scheduled once every six months over a two-year period to inspect, adjust, and service the ATT. The <br /> maintenance provider must submit an annual report and fee. Please review OAR 340-071-0345 for full <br /> requirements. <br /> TANK RISER: The tank must be installed with watertight risers extending to the ground surface or above. The <br /> riser lid must be gasketed and securely fastened. <br /> ELECTRICAL COMPONENTS ARE REQUIRED FOR INSTALLATION OF THIS SEPTIC SYSTEM. Detailed <br /> instructions for dosing tank/effluent pump installations are available in our office. The electrical wiring to the <br /> pump and alarm must be installed in accordance with the appropriate Electrical Code. The electrical work must <br /> be done under a Marion County Electrical Permit and must be inspected and approved by a Marion County <br /> Electrical Inspector. The work must be left uncovered and accessible for inspection. The electrical inspection <br /> includes the connections to the pump and alarm in the dosing tank,the wiring in the trench and the connections <br /> in the structure. <br /> HEADER PIPE must measure a minimum of 4 feet between Distribution Box and start of drainfield trench,must <br /> be installed level,and bedded on undisturbed earth. <br /> OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the DEQ rules. A copy of the <br /> DEQ setback requirements is on file in our office and available online at <br /> http://www.deq.state.or.us/wq/onsite/sdssearch.asp under Rules: Division 71:Table 1. <br /> DO NOT INSTALL SYSTEM DURING WINTER MONTHS(Nov.1—March 1) <br /> The geographical area in which the property lies is considered a risk to installation during winter months. <br /> Therefore,for this installation a Marion County Onsite Wastewater Specialist must specifically approve soil <br /> moisture conditions at this site for installation to occur during the period from November 1st to March 1st.
