The conditions observed during this site evaluation
indicate that the area around the test pit is capable of supporting a STANDARD system for the REPAIR. Plans are approved as
revised. Please refer to the signed approved site plan dated February 8,2019. Any deviation from the approved plans must be
authorized by this office prior to installation or the permit may be voided. It is the responsibility of applicant/installer to coordinate
these requirements with all others in OAR 340-071-0220.
This Permit is for a system designed to serve a single family dwelling with a maximum of 5 bedrooms. Peak sewage flow into
the system is limited to a maximum of 525 gallons per day, with an average sewage flow of not more than 263 gallons per day.
Premature failure of the treatment system may occur if either of these flow quantities is exceeded. If,for some reason, you
expect your domestic household water use may exceed these flows, it is advisable to increase the size of the treatment system.
Specific Requirements
TANK RISER: The tank must be installed with watertight risers extending to the ground surface or above. The riser lid must be
gasketed and securely fastened.
HEADER PIPE between Distribution Box and drainfield trench must measure a minimum of 4 feet, must be installed level, and
bedded on undisturbed earth.
OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the Department of Environmental Quality rules. A
copy of the DEQ setback requirements is available in our office and online at
http://www.oregon.gov/deq/Residential/Pages/Onsite.aspxunder Rules: Division 71:Table 1.
THE ATTACHED SEPTIC TANK ABANDONMENT FORMMUST BE COMPLETED AND RETURNED TO OUR OFFICE. The
septic system installer must complete and sign the form and attach a copy of the receipt from the septic tank pumping service
for pumping the tank.
DO NOT INSTALL SYSTEM DURING WINTER MONTHS(Nov.1—March 1)
The geographical area in which the property lies is considered a risk to installation during winter months. Therefore, for this
installation a Marion County Onsite Wastewater Specialist must specifically approve this site for installation from November 1 stto
March 1st. This may include a field visit directly prior to installation
THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTEDfrom any vehicular traffic, livestock and farming activities.
Other activities such as excavating or filling, driveway placement, paving or other similar activities that alter the natural soil
conditions will adversely affect the soil and the septic system function and lead to premature system failure. The area must not
be subjected to excessive saturation due to artificial drainage of ground surfaces, driveways, building downspouts or other
sources. The areas of the initial septic system and the future repair system must both be protected.
5/10/19:3:27:19PM Page 3 of 4 ONS_OnsiteCSC_pr
