The system consists of a 1000 gallon <br /> tank and 375 lineal feet of line,with a curtain drain upslope of the drainfield. The alteration will replace the existing tank and <br /> • expand the drainfield. <br /> Approved Area&System Type <br /> An evaluation of the test holes dug for soil analysis was conducted August 7,2017. The conditions observed during this site <br />-. evaluation indicate that the area around the evaluated test holes IS CAPABLE of supporting a STANDARD SEPTIC SYSTEM <br /> for the ALTERATION and future REPAIR. System plans are approved WITH NOTATIONS. Please refer to the signed - <br /> approved site plan revision dated February 9,2018. Any deviation from the approved plans must be authorized by this office <br /> prior to installation or the permit may be voided. It is the responsibility of applicant/installer to coordinate these requirements <br /> with all others in OAR 340-071-0220.• <br /> This Permit is for a system designed to serve a church. Peak sewage flow into the system is limited to a maximum of 1125 - <br /> gallons per day,with an average sewage flowof not more than 563 gallons per day. This is normally sufficient to serve a <br /> church with up to 225 in attendance.. Premature failure of the treatment system may occur if either of these flow quantities is <br /> exceeded. If for some reason you expect your water use may exceed these flows, it is advisable to increase the size of the <br /> treatment system. . <br /> Specific Requirements r <br /> COMMERCIAL SYSTEMSwith design flow greater than 600 gallons must be constructed in duplex(with two or more <br /> alternating pumps),withcontrols that activate an alarm when one of the pumps malfunctions. A pre-manufactured electrical <br /> control panel capable of monitoring pump performance through elapsed-time meters and cycle counters is required. An <br /> effluent filter and service access riser is required at the outlet of the septic tank. Service access manhole for the dosing tank <br /> must be adequate to construct, install,service and operate the equipment within. <br /> Dosing cycle must discharge no more than 20%of projected daily sewage flow each cycle(Maximum dose 260 gallons). A <br /> pre-manufactured electrical control panel is required. Elapsed-time meter and cycle counter is required to monitor pump <br /> performance. <br /> AN EFFLUENT LIFT PUMP WILL BE REQUIREDto install the septic system disposal field in the approved area and at the <br /> required depth. Detailed instructions for dosing tank/effluent pump installations are available in our office. The electrical wiring <br /> to the pump and alarm must be installed in accordance with the appropriate Electrical Code. 'The electrical work must be done <br /> under a Marion County Electrical Permit and must be inspected and approved by a Marion County Electrical Inspector. The <br /> work must be left uncovered and accessible for inspection. The electrical inspection includes the connections to the pump and <br /> - alarm in the dosing tank,the wiring in the trench and the connections in the structure. <br /> DRY SOIL CONDITIONS ARE REQUIRED FOR INSTALLATION(March 1 —October 31) The geographical area in which the <br /> property lies is considered a risk to installation during winter months. Soil moisture conditions generally preclude construction <br /> of the drainfield in the winter months. <br /> TANK RISERS MUST EXTEND TO THE GROUND SURFACE. The tank must be provided with watertight risers extending to <br /> - the ground surface or above. The riser lids must be gasketed and securely fastened. <br /> HEADER PIPE between Distribution Box and drainfield trench must measure a minimum of 4 feet,must be installed level,and <br /> bedded on undisturbed earth. - <br /> THEATTACHED SEPTIC.TANK ABANDONMENT FORMMUST BE COMPLETED AND RETURNED TO OUR OFFICE. The <br /> septic system installer must complete and sign the form and attach a copy of the receipt from the septic tank pumping service <br /> for pumping the tank. <br /> • <br /> OBSERVE ALL SETBACKS AND SEPARATION DISTANCESas required by the DEQ rules.
