Expiration date $19119 <br /> Work description NEW INSTALL 3 BDRM SFD <br /> s <br /> Conditions of approval <br /> Approved Area&System Type <br /> This property has been approved for an ALTERNATIVE TREATMENT TECHNOLOGY Treatment <br /> Standard 1 in the area identified in Site Evaluation Report 555-14-004953-SEP. A Capping Fill <br /> with Tile Dewatering is required for the drainfield. Proposed system stakeout has been approved <br /> and installation plans are approved as submitted. Refer to approved plans dated August 7, 2018. <br /> The installation must conform to these approved plans. Any changes or product <br /> substitutions must be approved by this office prior to installation. Any substantial changes <br /> may require complete revised plans with a plan review fee. Failure to comply may invalidate <br /> construction approval and permit. The required inspection schedule is attached to this permit. It's <br /> the responsibility of applicant/installer to coordinate these requirements with all others in OAR <br /> 340-071-0220. <br /> This approval is for a system designed to serve a single family dwelling with a maximum of 4 <br /> bedrooms. Peak sewage flow into the system is limited to a maximum of 450 gallons per day,with <br /> an average sewage flow of not more than 225 gallons per day. Premature failure of the treatment <br /> system may occur if either of these flow quantities is exceeded. If, for some reason, you expect <br /> your domestic household water use may exceed these flows, it is advisable to increase the size of <br /> the treatment system. <br /> Specific Requirements <br /> ATT Pretreatment Unit: AX20-RTN as per approved attached plan. <br /> SERVICE CONTRACTS: The owner of an ATT system must maintain a service contract with a <br /> maintenance provider certified by the manufacturer to serve and maintain the onsite system. A <br /> service contract must be entered into before the system is installed and must be maintained until <br /> the system is decommissioned. The service contract must provide for a minimum of four <br /> inspections and service visits by a maintenance provider scheduled once every six months over a <br /> two-year period to inspect, adjust, and service the ATT. The maintenance provider must submit an <br /> annual report and fee. Please review OAR 340-071-0345 for full requirements. <br /> TANK RISER: The tank must be installed with watertight risers extending to the ground surface <br /> or above. The riser lid must be gasketed and securely fastened. <br /> ELECTRICAL COMPONENTS ARE REQUIRED FOR INSTALLATION OF THIS SEPTIC <br /> SYSTEM. Detailed instructions for dosing tank/effluent pump installations are available in our <br /> office. The electrical wiring to the pump and alarm must be installed in accordance with the <br /> appropriate Electrical Code. The electrical work must be done under a Marion County Electrical <br /> Permit and must be inspected and approved by a Marion County Electrical Inspector. The work <br /> must be left uncovered and accessible for inspection. The electrical inspection includes the <br /> connections to the pump and alarm in the dosing tank, the wiring in the trench and the connections <br /> in the structure. <br /> HEADER PIPE between Distribution Box and drainfield trench must measure a minimum of 4 feet, <br /> must be installed level, and bedded on undisturbed earth. <br /> CAPPING FILL SYSTEMS MAY ONLY BE INSTALLED FROM JUNE 1ST TO OCTOBER 1ST or <br /> during such times when the Building Inspection Onsite Wastewater staff finds soil moisture <br /> conditions acceptable. See the attached"CAPPING FILL" information sheet for instructions. <br /> TILE DEWATERING system must be installed at a depth adequate to lower the saturation zone to <br /> below the drainfield trench. Tile dewatering trench must be constructed a minimum 12 inches <br /> wide,with a uniform trade of 0.2 to 0.4 feet of fall per 100 feet. Minimum trench depth is 43 inches <br /> with 31 inches of clean drain media or underdrain media (gravel) placed over a 4-inch perforated <br /> pipe (ASTM Specification F 405, D 2729 or F 810). Silt trap must have a 12-inch minimum <br /> diameter and effective trap area at least 12 inches below the invert of the outlet. Additional <br /> construction requirements are enclosed. <br /> OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the DEQ rules.
