" ct F � s^k ✓ '-� 'ami , ow <br /> T <br /> oi��l 14/ 4 T E lov �zL <br /> uwn70 * v1,44 0� 5€�:„ i <br /> s� <br /> � <br /> Conditions of Approval: <br /> Existing system information: <br /> The existing septic system serving this property was initially installed and approved in 1975,and includes a 1000-gallon septic <br /> tank and 375 linear feet of drainfield line.The system was designed to serve a three-bedroom residence. In 2014, an <br /> authorization/minor alteration was approved to add a 1500 gallon tank and pressure line to connect the hardship home to the <br /> existing septic system. <br /> The septic tank was most recently pumped by A&B Septic Service on January 22,2019. <br /> Picturessubmitted with this application indicate that the system is in satisfactory condition.There is adequate room for a <br /> replacement system on the property. <br /> Flow increase information: <br /> The existing septic system currently serves a three bedroom dwelling and a three bedroom hardship dwelling. This application is <br /> for continued use of a temporary hardship dwelling with different occupants. This is not an increase in use and meets the criteria <br /> listed under OAR 340-071-0205(7). This PERSONAL HARDSHIP RENEWAL TO CONTINUE CONNECTION INTO THE <br /> EXISTING ONSITE SYSTEM is approved. Please refer to the signed approved site plan dated April 26,2019. <br /> PERSONAL HARDSHIP RENEWAL is required every 5 years. <br /> THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTEDfrom any vehicular traffic, livestock and farming activities. <br /> Other activities such as logging,excavating or filling,driveway placement or any similar activities may also damage the soil in the <br /> area and adversely affect the septic system approval. The areas of the initial septic system and the future repair system must <br /> both be protected. <br /> The septic tank must be INSPECTED BY A LICENSED PUMPING SERVICE AT LEAST EVERY TEN(10)YEARS. Per OAR <br /> 340-071-0120(2)(c),the system must be evaluated for solids and pumped if over 35 percent or as needed. Excessive <br /> accumulation of these solid materials in the septic tank will cause the solids to wash out of the tank and into the drainfield. This <br /> will cause irreversible damage to the drainfield and will eventually lead to a system failure <br /> OBSERVE ALL SETBACKS AND SEPARATION DISTANCESas required by the DEQ rules. Parking areas must be located <br /> where vehicles will not impact any part of the system. A copy of the DEQ setback requirements is available in our office. <br /> Note:This Notice does not guarantee satisfactory or continuous operation of the sewage system. Should the system fail, a repair <br /> permit from DEQ is required. <br /> If you disagree with this report,you have the right to apply for an authorization notice denial review.The application for review <br /> must be submitted in writing within 45 days of the report issuance and be accompanied by the review fee in OAR <br /> 340-071-0140(3),Table 9C and any additional information DEQ needs to complete the review. <br /> You may apply for a variance to the onsite wastewater treatment system rules. The variance application must include a copy of <br /> the site evaluation report, plans and specifications for the proposed system,specify the rule(s)to which a variance is being <br /> requested, demonstrate the variance is warranted, and include the variance fee in OAR 340-071-140 Table 9C. A variance may <br /> only be granted if the variance officer determines that strict compliance with a rule is inappropriate or special physical conditions <br /> render strict compliance unreasonable, burdensome or impractical.
