EEi =OWL t k � h , <br /> T;°� ase t4, 41 0. <br /> Conditions of Approval: <br /> Existing system information: <br /> Marion County has no records regarding the installation of the septic system serving this residence.An Authorization notice to <br /> replace a 3 bedroom manufactured dwelling with a 3 bedroom manufactured dwelling was approved in 2002. <br /> The tank was most recently pumped and inspected by Oregon Sewer&Drain LLC on March 29,2019. The technician reported <br /> that there was a 1000 gallon tank functioning properly.The drainfield was also located and evaluated at that time.The technician <br /> located 140 feet of drainline by using an electronic locator and physically uncovering it.The technician reported that the- <br /> distribution box was damaged. <br /> A site visit was conducted on April 23,2019 in response to an application to replace the existing 3-bedroom manufactured <br /> dwelling with a new 3-bedroom dwelling. The system was partially exposed for inspection. There were no signs of failure <br /> observed.The distribution box was repaired. There is adequate room for a repair system. <br /> Flow increase information: <br /> According to the DEQ rules for onsite wastewater treatment systems,THIS AUTHORIZATION TO CONNECT THE <br /> REPLACEMENT DWELLING TO THE EXISTING ONSITE SYSTEM IS APPROVED under Oregon Administrative Rule(OAR) <br /> 340-071-0205(4). Please see the approved signed site plans dated April 24,2019. <br /> THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic including construction vehicles, <br /> livestock and farming activities. Other activities such as logging,excavating or filling,driveway placement or any similar activities <br /> may also damage the soil in the area and adversely affect the septic system approval. The areas of the initial septic system and <br /> the future repair system must both be protected. <br /> The septic tank should be pumped and inspectedby a licensed septic tank pumping service approximately every five(5)years. <br /> The septic tank is a reservoir for the solid materials in the sewage waste stream. Excessive accumulation of these solid <br /> materials in the septic tank will cause the solids to wash out of the tank and into the drainfield. This will cause irreversible <br /> damage to the drainfield and will eventually lead to a system failure. <br /> Note:This Notice does not guarantee satisfactory or continuous operation of the sewage system.Should the system fail, a repair <br /> permit from DEQ is required. <br /> If you disagree with this report,you have the right to apply for an authorization notice denial review.The application for review <br /> must be submitted in writing within 45 days of the report issuance and be accompanied by the review fee in OAR <br /> 340-071-0140(3),Table 9C and any additional information DEQ needs to complete the review. <br /> You may apply for a variance to the onsite wastewater treatment system rules. The variance application must include a copy of <br /> the site evaluation report,plans and specifications for the proposed system,specify the rule(s)to which a variance is being <br /> requested,demonstrate the variance is warranted, and include the variance fee in OAR 340-071-140 Table 9C. A variance may <br /> only be granted if the variance officer determines that strict compliance with a rule is inappropriate or special physical conditions <br /> render strict compliance unreasonable, burdensome or impractical.
