The system consists of a 1000 gallon concrete dosing septic tank with 375 lineal feet of EZ flow drainline with <br /> pressure operated equal distribution via hydrosplitter. Design flow is 450 gallons per day,generally adequate for a single family <br /> dwelling with up to four(4)bedrooms. A Certificate of Satisfactory Completion is on file. <br /> The septic tank was not pumped or evaluated due to it only being in use since 2018. <br /> The site plan shows the proposed structures and concurrent site development can maintain all required setbacks to the existing <br /> system. Placement of a three-bedroom single family dwelling will not exceed the design flow of this system. The design of the <br /> existing septic system at this address is to serve a single family dwelling of up to four bedrooms,with a maximum <br /> daily sewage flow of 450 gallons. This permit DOES NOT infer additional living space and DOES NOT provide for <br /> additional daily sewage flow. <br /> Flow increase information: <br /> According to the DEQ rules for onsite wastewater treatment systems,the proposed increase in projected daily flow,to replace <br /> the existing two bedroom single family dwelling with a three bedroom single family dwelling,meets the criteria listed in OAR <br /> 340-071-0205(5)(a)-(d)and is an allowable increase for this system.This AUTHORIZATION TO CONNECT TO THE EXISTING <br /> ONSITE SYSTEM is approved. Please refer to the signed approved site plan dated April 16,2019. <br /> THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic including construction vehicles, <br /> livestock and farming activities. Other activities such as logging,excavating or filling, driveway placement or any similar activities <br /> may also damage the soil in the area and adversely affect the septic system approval. The areas of the initial septic system and <br /> the future repair system must both be protected. <br /> • <br /> THE SEPTIC TANK SHOULD BE PUMPED AND INSPECTED by a licensed septic tank pumping service when sludge and <br /> scum accumulation reaches 30%of tank volume(see attached pumping recommendations). Pumping every 3-5 years is <br /> average. The septic tank is a reservoir for the solid materials in the sewage waste stream. Excessive accumulation of these <br /> solid materials in the septic tank will cause the solids to wash out of the tank and into the drainfield. This will cause irreversible <br /> damage to the drainfield and will eventually lead to system failure. <br /> Note:This Notice does not guarantee satisfactory or continuous operation of the sewage system. Should the system fail,a repair <br /> permit from DEQ is required. <br /> If you disagree with this report,you have the right to apply for an authorization notice denial review.The application for review <br /> must be submitted in writing within 45 days of the report issuance and be accompanied by the review fee in OAR <br /> 340-071-0140(3),Table 9C and any additional information DEQ needs to complete the review. <br /> You may apply for a variance to the onsite wastewater treatment system rules. The variance application must include a copy of <br /> the site evaluation report, plans and specifications for the proposed system,specify the rule(s)to which a variance is being <br /> requested,demonstrate the variance is warranted, and include the variance fee in OAR 340-071-140 Table 9C. A variance may <br /> only be granted if the variance officer determines that strict compliance with a rule is inappropriate or special physical conditions <br /> render strict compliance unreasonable, burdensome or impractical.
