Please refer <br /> to the REVISED signed approved site plan dated February 8,2017. Any deviation from the approved plans <br /> must be authorized by this office prior to installation or the permit may be voided. <br /> This Permit is for a Holding Tank designed to serve a commercial building with a maximum of thirteen <br /> employees.Daily sewage flow is limited to 200 gallons or less. <br /> Specific Requirements <br /> HOLDING TANK: 2000 gallons TRAFFIC RATED <br /> HOLDING TANK MUST COMPLY WITH OAR 340-071-0340. <br /> Anti-buoyancy is required as per manufacturer's specifications. <br /> AUDIBLE AND VISIBLE ALARMS set to indicate when the tank is 75 percent full. Only the audible <br /> alarm may be user cancelable. The electrical wiring to the alarm must be installed in accordance with the <br /> appropriate Electrical Code. The electrical work must be done under a Marion County Electrical Permit <br /> and must be inspected and approved by a Marion County Electrical Inspector. The work must be left <br /> uncovered and accessible for inspection. The electrical inspection includes the connections to the alarm in <br /> the tank,the wiring in the trench and the connections in the structure. The Alarm float must be positioned <br /> in the holding tank to signal the audible and visual alarm when liquid level is at seventy five (75%) of <br /> storage capacity for scheduling pumping services. <br /> OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the DEQ rules. A <br /> copy of the DEQ setback requirements is included for your review. <br /> OPERATION AND MAINTENANCE. At all times the holding tank is being used,the owner ofthe tank <br /> must maintain a service contract with a sewage disposal service licensed under OAR 340-071-0600 to <br /> provide for regular inspection and pumping of the holding tank. <br /> ANNUAL REPORT. Owner of holding tank(s)must submit an annual report along with a fee. <br /> THIS SYSTEM MUST BE INSPECTED AND APPROVED PRIOR TO BACKFILL. When work is <br /> complete and ready for inspection,request an inspection by following the instructions on the accompanying <br /> permit document. Be sure the System Record form with the As-Built drawing is on the job or in our office <br /> when the inspection is requested.
