STATEMENT DF C0~'LIANCE Date: S~PTEMBEk 11, 1997 Contract Nu~ber: I, TR~Y 4JRI6HT, GFFICE PWNAGER do hereby state: fl) That I pay or supervise the pay~ent of the persons e~played by STATQN EQUIPMENT f~NTRLS on the ~ CO/DEMtI/SRLEM, that during the payroll period co~~enring on the 31 day of pUC~1ST, 1997 and ending the 6 day of S~MAER, 1997, all persons erployed on said project have been paid the full weekly wages earr~ed, that no rebates have beer or will be ~ade either directly or indir,ectiy to or un behalf of said STATON EQUIRMENT RENTALS fror the full Weekly wages earned by any person and that no deductions have been ~ade either directly or indirectly fro~ the full wages earned by any person, other thar~ per~l551b1~ dedurtions as defined in Regulations, Part 3!29 CFA Subtitle A), issued by the Secretary of Labor under the Copeland Act, as a~ended (48 Stat. 948.6s Stat. 1@8, 72 Stat. 9E7; 7b Stat. ,~,7; wl~ U.S.C. 27bc), and described below: !2) That any payrolls otherwise under this contract required to be sub~itted for the above period are correct and cr,epiete; that the wage rates for labarers or ~echanics cantained therein are not less than the applicable Hage rates contained in any wagE deter~ir~ation incorporated into the cantract; that the classifications set forth therein for each laborer or rechanic confarr with tf~e -vork he perfar~ed. t3) That any apprentices eeployed in the above period are duly registered in a bona fide apprenticeship progra~ registered aith a State apprenticeship agency reragnized by the Rureau of Apprenticeship and Training, United States Depart~ent of Labor, or if no such recognized agency exists in a State, are registered with the Aureau of Apprenticeship and Training, United States Depart~ent of Labar. !4) That: !a) I~RE FRIt~E AENEfI1S ARE PAID TU APPR(1VED PLf~IS, FIkJDS, OR PR~6RAM5 (%l - In addition ta the basic hourly Nage rates paid to each laborer or ~echanic listed in the abave referenced payroll, pay~ents of fringe benefits as listed in the cantract have been or will be ~ade to appropriate progra~s for the benefit of such e~ployees, except as noted in Sectian 4tc) below.
