~ Instructions for Filing DEQ Forcn ASN-6 Notice for Removal of Non-friable Asbestos What notice is required? Under Oregon law, written notification must be given to the DepaRment of Environmental Quality (DEQ) for removal of aon-friable asbestos {OAR 340-32-5620(4)}. Notice must be submitted on forms provided by the Department and must be accompanieci by the required Notification Fe~. What ~re the deadlines for notiFying DEQ? How to f U out the form. Be sura you complzte all tines that apply to your project. Here are some guidelines for providing tha requirecl information: Is this a revision to a notiPcation? Check "yes" if this is an amendm~nt to a previous notice. Check "no" if this is a new notice and a naw fee. DEQ must receive written notification no less than 5 days prior to the starting date of the asbestos abatement projact. The 5-clay Advance Notification Period will not be~in until thz fully completed ASN-6 Form and tha Notification Fee have be~n received bythe Department. You will not receive a confirmation of receipt from DEQ. Keep a copy of the completecl form for your files. Unless notified otherwise, you may begin your project on the scheciuled starting date. What if there ure changes in the project? DEQ must bc aotifiecl in writing if there are projact changes. Th~ simplest way to do this is to send DEQ a copy of the original notice, highlighting or circling all changrs. If you fill out a new Form ASN-6, all the revisecl sections must still ba highli~hted or circleci so they can be identified. Can the form be FAXED to DEQ? Don't use FAX machines to sand original notices. Send original notification to address balow. You may send amended notification forms by FAX. If sending by fax, circl~ all changes, instead of highlighting them. See the asbestos contact information sheet for the appro~riate DEQ FAX aumbers. What about the Notification Fee" Send a check for the Notification F~e whea you send the original notice to DEQ. Make the check payable to "DEQ" or "Department of Environmental Quality." For emergency notifications, the amount of the fe~ is increased by 50 p~rcent. Descriptions of the facility, the type of asbestos material, the quuntity und the method of removal or encapsulation: Be sp~citic and completa in providing this information. Attach to th~ fortn additional information on speciat work practicas or disposat tachniyues which require prior approval from DEQ. COntraCtor: Giv~ the name, address, telephone number and CCB registration (if applicabla) number of the contractor for ~ the project. Days of week und hours of day to be worked (work shift): List th~ days of the week and the hours of the day that abatement will take place. Property Owner: Giva tha name and address of the property owner. Site Contact: Giv~ the nama and telephone number of the person to contact regarding the job site. Disposal site and waste hauler: Be specific and complete. Signing the form: Indicate th~ name and phone number of the property owner, oparator or abatament contractor complating the form.
